근치적 신절제술 후 방광에 전이된 신세포암

Abstract
신세포암은 폐나 림프절 등의 장기에는 흔히 전이하는 질환이지만, 방광으로 전이되는 경우는 극히 드물다. 신세포암의 방광 전이는 전세계적으로 1997년까지 32례가 보고 되었고, 이후 3례가 추가로 보고되어 현재까지 35례 정도가 보고되었다.1 국내의 경우는 2례의 증례 보고가 있었으며, 1례는 신세포암 발견 당시 전신에 원격전이가 동반된 사례이었고, 다른 1례는 신세포암이 12x7.5cm로 신우와 신배를 침범한 소견이 있었다. 저자들은 T1N0M0의 낮은 병기를 보이는 환자에서 좌측 신세포암으로 근치적 신절제술을 받은 후 특별한 문제없이 지내다가 2년 후에 신세포암이 방광과 우측 기관주위 림프절로 전이된 예를 경험하였기에 보고하는 바이다.
We report a case of metastatic renal cell carcinoma to the urinary bladder. A 61-year-old man presented with gross hematuria. The patient had been treated with a radical nephrectomy at 59 years of age, for renal cell carcinoma. Cystoscopy revealed two polyploid tumors protruding into to the urinary bladder. A transurethral resection was performed and the pathological diagnosis of the lesion in the urinary bladder was a renal cell carcinoma. Metastases to the paratracheal lymph nodes were confirmed on the chest computed tomography. The patient was treated with immunotherapy(5-fluorouracil, interferon-a, interleukin-2) for 20 weeks.
