Abstract
목적: 원인이 확실하게 알려져 있지 않은 조기폐경의 발생기전에 apoptosis가 관련되는 지를 규명한다.
연구 방법: 17명의 조기폐경 환자에서 복강경하 난소생검을 시행하여 H-E 염색하에서 조직학적 특징을 분석하고, apoptosis관련 단백질인 bcl-2, bax, fas와 fas-L의 발현을 알아본다.
결과: 낭종성폐쇄난포(cystic atretic follicle)은 17명중 4명의 환자에서, 백체는 1명중 9명의 환자에서 그리고 정상폐경 환자에서는 발견되지 않는 원시난포(primordial follicle)는 17명중 3명에서 관찰되었다. Bcl-2에 대한 면역조직학적염색에서 원시난포의 전과립세포(lining cell)에만 약하게(+) 염색이 되었으며, 난포세포에는 염색이 되지 않았다. 낭종성폐쇄난포(cystic atretic follicle)와 간질에도 염색이 되지 않았다. Bax에 대한 면역조직학적 염색에서 원시난포(primordial follicle)의 난포세포(oocyte)와 전과립세포(lining cell) 모두에서 중등도로(++) 염색이 되었따. 그리고, 낭종성 폐쇄난포에서도 중등도로(++) 염색이 되었으나, 간질에서는 염색이 되지 않았다. Fas에 대한 면역조직학적 염색에서 원시난포(primordial follicle)의 난포세포(oocyte)와 전과립세포(lining cell) 모두에서 강하게(+++) 염색이 되었고, 낭종성 폐쇄난포와 간질에서는 중등도로 염색이 되었다. Fas-L에 대한 면역조직학적 염색에서 원시난포(primordial follicle)의 난포세포(oocyte)와 전과립세포(lining cell) 모두에서 강하게(+++) 염색이 되었고, 낭종성폐쇄난포에서는 약하게(+), 간질에서는 중등도로(++) 염색이 되었다.
결론: 조기폐경환자의 난소에서 apoptosis관련 단백질이 발현되는 것으로 보아 조기난소부전의 발생에 apoptosis가 관련되며, bax, fas와 fas-L 등이 조기폐경에서의 조기난포소실을 유발한다.
Objectives: Recently, it was suggested that apoptosis may be a basic mechanism of follicular atretic process. If the POF(premature ovarian failure) results from an acceleration of the process of atresia of the oocytes causing premature deletion of stored oocytes, POF may be a good model for the apoptosis. Our objective is to determine the relation between the POF and apoptosis.
Methods: We performed laparoscopic ovarian biopsy in 17 patients with POF. We evaluated the ovaries from a patient with POF by conventional H-E stirring and immunohistochemical staining for apoptosis related protein(bcl-2, bax, fas and fas-L).
Results: Atretic cystic follicles were seen 4 out of 17 patients, and corpora albicans indicating past ovulation
were seen 9 out of 17 patients. Primordial follicles were seen in 3 out of 17 patients, however developing follicles were not seen in all patients. Immunohistochemical analysis revealed that Bax was expressed in the lining cells of the atretic cystic follicle and primordial follicle and mesothelial cells lining the surface of the ovary. Fas and Fas-L were expressed on the lining pregranulosa cells and primary oocyte of the primordial follicle in the ovary of the patient with premature ovarian failure. Bax were also expressed on the lining pregranulosa cell and oocyte of the premature ovarian failure. Bcl-2 was expressed in the lining cells of primordial follicles of the patients with POF.
Conclusion: Apoptosis may be a basic mechanism of POF. Bax, fas and fas-L may play a role causing premature follicular depletion.
