Abstract
서 론
악성 또는 역형성(anaplastic)수막종은 전체 수막종의 약 2∼12%를 차지하는 것으로 알려
져 있으며, 그 발생 부위는 거의 대부분 천막상부로서 약 70%가 궁융부(convexity)나 방시
상동(parasagittal)이고 후두와중 특히 추체골에서 발생하여 두개강외로 침범한 경우는 매우
드물다.
본 교실에서는 추체골에서 발생하여 소뇌교각부와 하측두와(infratemporal fossa)로 진출
하면서 내경정맥을 침범한 희귀한 악성(유두상) 수막종을 두개강 내외를 통한 여러 수술적
접근 방법을 이용하여 제거한 예를 경험하였기에 고찰과 함께 보고하는 바이다.

The authors present a case of malignant papillary meningioma of the petrous bone
involving the whole petrous bone which extended to the left cerebellopontine angle and
internal jugular vein extracranially.
A 46-year-old male patient was presented with a 3-years history of the left otalgia
and subsequent deafness with left facial nerve palsy. Neurological examination revealed
a complete palsy of the left eights nerve, and incomplete palsies of the left 7th, 9th,
11th, and 12th cranial nerves.
Magnetic resonance imaging and computerized tomographic scan demonstrate a huge
mass in cerebellopontine angle which involved the whole petrous bone, suboccipital bone
and internal jugular vein.
The tumor was totally resected using the transjugular approach with modified petrous
bone resection. Neuropathologic examination revealed a papillary meningioma.
