Abstract
서 론
미세신경외과 수술 및 신경마취학의 발전으로 뇌동맥류 수술에 따르는 합병증 및 사망율
은 현저히 저하되고 있지만 아직도 뇌동맥류의 크기, 위치, 해부학적 변이로 인하여 접근하
기 어려운 뇌동맥류의 수술은 신중하게 수술법을 선택해야 할 대상이다
대개 뇌동맥류의 결찰과 일시적 결찰시 뇌 보호를 위해 Barbiturate, 순환정지, 초저체온
법 등의 방법이 사용되나, Barbiturate만으로는 뇌보호시 모동맥 일시적 결찰 최대허용시간
은 약 15분 정도라는 한계가 있다. 거대뇌동맥류. 뇌간에 발생한 혈관종과 뇌동정맥 기형 등
의 수술에서와 같이 비교적 장기간 혈류의 차단이 요구되는 상황에서 효율적인 뇌보호를 위
해 초저체온법과 순환정지술을 사용하게 된다.
본 교실에서는 서혜부 절계와 대퇴동적맥 도관을 이용한 순환정지와 초저체온법으로 우측
중대뇌 거대동맥류 결찰을 성공적으로 시행한 증례를 보고함과 아울러, 뇌동맥류 수술에초
저 체온법과 순환정지술의 적용을 고찰하고자 한다.

Despite significant advance in neurosurgical and neuroanesthesic technique, certain
lesions such as giant aneurysms still present significant challenge to neurosurgeons.
The authors experienced a case of giant MCA bifurcation aneurysm surgery using a
technique of deep hypothermia and circulatory arrest with excellent outcome.
Cannulation of cardiopulmonary bypass can be done either centrally through a
stern-otomy(aortic-right atrial) or peripherally through a small inguinal incision
(femoral-femoral).
In our case, femoral-femoral cannulation was done which can eliminate many serio-us
complications following the sternotomy.
The rationale for the application of the deep hypothermia and circulatory arrest in the
complex intracranial aneurysm surgery and the benefits of the femoral cannulation are
discussed.
