Abstract
서 론
제 1경추-제 2경추 경관절 나사못 고정술(C1/2 transarticular screw fixation)은 1970년대
Magerl이 처음 시도한 이래 많은 발전과 경험이 보고되고 있다. 제 1경추-제2추 경관절 나
사못 고정술은 상부경추 불안정증때 후두골와 제 1경추가 담당하는 운동을 제한하지 않으면
서 불안정한 관절만 즉시 고정하고 생체역학적으로 다른 방법보다 견고하여 골유합율이 더
높다. 특히 Os odontoideum은 전방전위성이 심하여 제 1경추-제 2경추 경관절 나사못 고정
술로 정복 및 고정효과가 크며 상부경추 신경관 협착증으로 후궁절제술을 시행해야 할 때나
경추후궁에 결핍이 있을 때 관절간 골유합술을 시행할 수 있어서 매우 유용하다.
본 저자는 최근 복잡한 Os odontoideum 3예를 제 1경추-제 2경추 경관절 나사못 고정술
로 치료하고 경험을 참고 문헌 고찰과 함께 보고한다.

C1/2 posterior transarticular screw fixation has become an accepted method of rigid
internal fixation for patients requiring posterior C1/2 fusion. Especially, Surgery for Os
odontoideum can be troublesome due to its severe translational instability, canal stenosis
requiring decompressive laminectomy and defective C1 posterior arch.
The author reports three cases of complicated Os odonoideum(one cases of failed
occipito-C3 fusion, two cases requiring C1 laminectomy) with C1/2 transarticular screw
fixation and interarticular bone fusion. All required decompressive laminectomy and
demonstrated highly unstable translational instabilities.
On the basis of these experience, it is postulated that C1/2 transarticular screw
fixation seems to be superior to other methods in stabilization and fusion for
complicated Os-odontoideum.
