Abstract
서론
자기공명영상에 의하여 뇌간의 출혈을 일으키는 병변 중 해면상혈관종은 비교적 용이하게
진단할 수 있게 되었다. 뇌간에 발생하는 병변은 수술적 접근이 어려우나, 일부 경우에 선택
적으로 수술적 접근이 효과적 일 수 있으며, 수술 중 전기생리적인 감시에 의하여 수술의
안전성을 높이고 효과적인 접근을 할 수 있다 뇌간의 해면상혈관종에 대한 수술적 접근은
1971년 Lassiter등인 처음으로 보고하였다. 자기공명영상 및 미세수술의 발전으로 수술 후
합병증 및 사망률은 급격히 감소하였고, 특히 수술 중 체성감각 및 청각 유발전위와 제 4뇌
실의 도식화(mapping)를 통한 안면신경, 설하신경의 뇌간내 경로의 확인은 수술 후 신경결
손을 줄이는데 크게 도움이 된다.
저자들은 뇌교에 발생한 해면상혈관종 2예의 수술을 시행하면서 수술 중 체성감각 및 청
각 유발전위와 단극자극에 의한 제4 뇌실의 도식화를 시행함으로써 성공적으로 제거 할 수
있었기 에 문헌고찰과 함께 보고하는 바이다.

In general, surgical approaches to intrinsic lesions of the brain stem are problematic.
We encountered two cases of pontine cavernous hemangioma in which successful
removal was possible with the help of intraoperative electrophysiological monitoring.
During surgery, somatosensory and auditory evoked potentials, were subject to
intraoperative bimodal monitoring. Before the removal of hemangiomas, monopolar
stimulation and monitoring of the facial EMG identified facial colliculus on the floor of
the fourth ventricle. During and after surgery, there was some increase in the amplitude
of BAEP and SSEP, and the authors recognized that post-surgical E-P information
correlated well with surgical outcome. We observed marked improvement of preoperative
neurological deficits and follow-up brain MRI showed that the hemangiomas had been
completely removed.
These favorable results demonstrate the usefulness of intraoperative
electrohphysiological monitoring, and indicate the appropriacy of more aggressive surgical
treatment of symptomatic cavernous hemangioma of the brain stem.
