두부외상 후 발생한 내경동맥 손상에 의한 뇌경색증 1례

Abstract
결론
혈종 형성이 없는 두부 외상 환자에서 신경학적 이상 소견 없이도 뇌기저 골절이나 경추
체 골절 혹은 이와 연관된 추체부나 경부 내경 동맥의 손상에 대한 면밀한 주의나 감시가
요할 것이며, 이러한 손상이 의심되는 경우는 즉각적인 뇌혈관 촬영으로 빠른 시일 내에 병
변의 본체나 부위를 정확히 확인하여 수술적인 시기를 놓치지 않는 것이 중요할 것으로 사
료된다
두부외상 후 초기 뇌전산화단층촬영상 기저골 골절 등의 뇌손상으로 치료 중에 의식의 악
화와 새로운 신경학적 결손징후가 있어 추적검사한 뇌 전산화 단층 촬영에서 뇌경색의 소견
이 보였고, 뇌혈관 촬영 시행 결과 추체부 내경동맥의 손상으로 인한 내경동맥 근위부의 폐
색소견과 중대뇌 동맥기시부의 색전증에 의한 폐색 소견 확인하였다. 뇌 전산화 단층 촬영
상 이미 저음영이 광범위하고 정중선 이동이 상당히 진행된 단계라 혈관에 대한 직접적인
접근 못하고 두개 감압술 및 뇌엽절제술 등 실시하였으나 소생하지 못한 례를 보고하고 두
부 외상 후 발생한 혈관 손상에 대한 조기 인지의 중요성을 아울러 알리는 바이다.

Deteriorated mental status and new neurological deficits after head trauma signify the
formation of intracranial hematoma or brain edema. In a case of head trauma
accompanied by basal skull fracture, even if without intracranial hematoma formation or
edema, we should be alert to the possibility of injury of the petrosal carotid artery
which needs prompt diagnostic evaluation and management.
We report a case of traumatic cerebral infarction due to injury of petrosal internal
carotid artery and review the literatures.
