Abstract
서론
거대 사형 동맥류(Giant serpentine aneurysm)는 1977 & Mclaurin에 의하여 처음으로 명
명된 이후 현재까지 64례가 보고되어 있을 정도로 희귀하며 국내에서는 그 증례 보고를 쉽
게 찾을 수 없는 실정이다 또한 발생부위에서 거대 사형 동맥류는 중대뇌 동맥에 호발하지
만, 전교통 동맥에 발생된 경우는 처음으로 추정된다.
거대 사형 동맥류는 그 특징상 크기가 거대하고, 동맥류내에 각기 다른 혈전으로 차 있다.
뿐만 아니라 동맥류의 경부가 없으며, 혈류의 유입부와 유출부가 서로 다른 특징이 있고, 정
상적인 혈관벽을 갖지 않은 뱀모양의 꾸불꾸불한 사형의 혈관통로를 갖고 있다 따라서 일반
적인 거대 낭상이나 방추형 동맥류와는 임상적 특징 및 방사선학적 소견이 상이하여 거대
동맥류 중 독특한 형태의 소그룹으로 구분되고 있다.
저자들은 전교통 동맥에 발생한 거대 사형 동맥류를 치험하고 그 발생기전 임상적 특징,
방사선학적 소견, 감별진단 및 치료 등에 대하여 문헌고찰과 함께 보고하고자 한다.

Giant serpentine aneurysm(GSA) is a giant aneurysm containing a serpentine channel
which enters and exits at separate sites and is continuous with the parent vessel.
This 33-year-old man with a two-year history of frontal headache presented with
bitemporal hemianopsia three months before admission. Supraseller mass of computed
tomography revealed a giant(3×2.5×2cm) mixed density. mimicking a brain tumor.
Cerebral magnetic resonance images showed a flow void within the mass, and this was
consistent with the serpentine vascular channel demonstrated by angiography.
The aneurysm was revealed by surgery involving the frontobasal interhemispheric
approach, left A1 & A2 were clipped and the mass of the aneurysm was removed in
toto an aneurysmorrhaphy was constructed, and to preserve the distal blood flow to the
left A2, the patients made an unevenful recovery.
