Abstract
서론
역사적으로 뇌수두증의 예후는 매우 나빠서 사망율이 80%에 달했으나, 단락술의 도입과
일반적인 내과적 치료의 발달로 지난 30년간 예후의 의미 있는 호전이 있었다. 그러나 아직
도 단락술후 합병증으로 인하여 예후가 나빠지는 경우가 있다. 단락술후 합병증은 매우 다
양하나 크게 두 가지를 들면 단락관이 막히거나 또는 감염이 발생하는 것이다. 또한, 소아기
때에는 성장하면서 단락관이 끊어지는 경우도 있다. 말단 단락관의 위치에 따라 수술방법을
뇌실-복강간 단락술과 뇌실-심방간 단각술로 나누는데 이중 뇌실-심방간 단각술후 만성적
으로 감염되어 있다가 신우염이 합병증으로 동반되는 경우 이를 단락 신장염(shunt
nephritis)이라 하며 1965년 black에 의해 처음 보고되었다. 신우염은 보체계(Complement
system)을 활성화시키는 면역 복합체(immune complex)가 사구체에 침착함으로써 발생하게
된다. 치료는 감염된 단락관을 제거하고 장기간 항생제 투여를 하는 것이다. 본 교실에서는
뇌실-심방간 단락술 후 표피포도 상구균(staphylococcus epidermidis)에 의해 감염되어 발생
한. 단락술 감염의 합병증인 단락신장염 1례를 경험하였으며 국내에서 발생한 첫 사례로 생
각되며 문헌 함께 보고하는 바이다.

A 8-year old girl with a ventriculoatrial shunt presented with fever and gross
hematuria. Staphylococcus epidermidis was cultured from blood. Light electron
microscopic findings in this patients were characteristic of shunt nephritis and serum
complement levels were low, After shunt removal, hydrocephalus was so severe that
ventriculoperitoneal shunt revision was performed opposite Kocher's point ; continuous
antibiotic therapy resulted in normalization of the clinical symptoms. A review of the
literature is included.
