Abstract
서론
결절성 경화증은 지능저하(mental retardation), 간질(seizure), 피하선종(adenoma
sebaceum) 등 여러 장기들의 다양한 증상들을 임상적 특징으로 하는 매우 드문 유전질환이
다. 병리학적으로는 먼저 뇌신경계에서 하피질부. 백질, 기저핵 부위에 여러 개의 경화결절
(sclerotic nodule)과 측뇌실 주변부에 석회화(calcification)병변 또는 상리하 거대성상세포종
(subependymal giant cell astrocytoma) 등이 나타나며, 피부 병변으로 안면부 피지선종
(adenoma sebaceum), 요천추부의 샤그린 반점(shagreen patch), 그리고 저색소성 또는 탈색
소성 모반(hypopigmented or depigmented nevi)이 피부 전체에 나타날 수 있다 그 외 다른
장기에도 병변이 출현할 수 있는데, 안구의 모반(phakoma) 또는 시신경교종(optic nerve
glima), 신장의 맥관지방근유종(angiomyolipoma) 또는 낭종(cyst), 심횡문근종(cardiac
rhabdomyoma), 폐의 낭성질환 간의 혈관종(angloma) 또는 과오종(harmatoma) 보일 수 있
고, 두개골, 척추, 골반뼈 등의 골격계에도 화증(osteosclerosis) 또는 낭성변차(Cystic
change)가 보일 수 있는 등 여러 장기에서 다양한 임상 증상이 발한 있다. 유사질환인 다낭
신(polycystic kidney)의 경우에는 두개강내 뇌동맥류의 높은 출현빈도를 보이지만, 경화증
의 경우에는 동맥의 이상소견이 신혈관에서 발견되나, 두개강내 뇌동맥류는 전세계적으로
지금까지 10여례만 보고되고 있다 저자들은 최근 결절성 경화증을 가진 젊은 성인 남자에
동반된 후하소뇌동맥류 1례를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

Tuberous Sclerosis is a hereditary connective tissue disorder affecting multiple organs
and is associated with a wide variety of abnormalities of the central nerve system. The
symptoms vary according to the organ involved and may appear in later life. In
connective tissue disorders, vascular anomalities are frequent, though cerebrovascular
anomalies are extremely rare. We recently, encountered a case of posterior inferior
cerebellar artery aneurysm associated with tuberous sclerosis. This report describe
clinical and radiological details of this case and includes a brief review of the literature.
