Abstract
서론
간세포암은 전세계적으로 발생률이 높은 종양중의 하나로 특히 아시아의 남부 아프리카
발병율이 매우 높은 것으로 보고되고 있으며, B형 간염의 유병율이 높은 우리 나라에서도
간세포암의 발생 빈도가 매우 높고 그로 인한 사망률도 매우 높다 간세포암의 치료 방법중
의 하나인 간동맥을 경유한 화학색전술에 대한 치료 효과에 대하여는 환자의 생존 기간을
증가시킨다는 많은 문헌 보고가 있다.
화학색전술은 절제하기 어려운 간세포암 환자에서 용이하고 효과적인 치료방법으로 사용
할 수 있으나 전체 시술한 환자의 10%이내에서 여러 가지 합병증이 발생할 수 있으며,4∼
8%는 감염과 관련된 합병증으로 보고되고 있다
농양이 발생된 경우는 주로 간이나 담낭 및 담도 관계에 발생하며, 간동맥 화학색전술과
연관되어 뇌에 발생한 농양은 매우 드물다. 저자들은 간동맥을 경유한 화학색전술을 시행한
후 뇌 측두엽에 발생한 뇌 농양을 치험하였기에 문헌 고찰과 함께 보고하는 바이다.

We describe a case of brain abscess which developed in a 64-year-old man after
hepatic artery chemoembolization(HACE) for hepatocellular carcinoma(HCC). On his first
admission, percutaneous liver biopsy led to a diagnosis of type-C liver cirrhosis, he
readmitted 5 years later and HCC was diagnosed. The patient underwent two HACE
procedures, with a two-month interval. Three month later, he was readmitted to hospital
with headache and brief loss of consciousness ; brain magnetic resonance imaging(MRI)
revealed a brain abscess. Stereotactic aspiration was performed and antibiotics sensitive
to the klebsiella pneumonia bacteria cultured in the abscess were administrated
intraveously. On month later, the abscess size became smaller and the patient was
discharged. This report reviews the literature and discusses the pathogenesis of brain
abscess after intra-arterial embolization.
