Abstract
서론
뇌동맥의 fenestration은 한 동맥의 내강(lumen)이 나뉘어져 두 개의. 내면이 내피로 덮힌
통로(endothelium-lined channel)가 초래된 기형을 일컫는데, 모양이 짧은 것부터 긴 부분에
걸친 중복(duplication)까지 다양하다. 전교통동맥 이외 뇌동맥의 fenestration은 비교적 드문
선천적 기형으로서 척추-기저동맥에 호발되며, 더욱 드물게 Al, Ml, 또는 내경동맥에도 발
생된다. 뇌동맥의 fenestration은 흔히 무증상의 우연한 소견으로 발견되지만, fenestration
자체, 주로 fenestration의 근위단에 동맥류가 발생되거나 다른 뇌동맥에 동맥류가 동반된
경우들이 보고되었다. Sanders등은 5190건의 뇌혈관 조영 보고서를 조사한 결과 37예
(0.71%)에서 38개의 뇌동맥 fenestration이 발견되었는데, 이중 16개가 기저동맥에, 10개가
척추동맥에. 9개가 중뇌동맥에, 그리고 3개가 전뇌동맥에 위치하였고, 이들 중 하나의 기저
동맥 fenestration에서만 동맥류가 동반되어 fenestration 자체에 동맥류가 발생된 율은 3%
이었다고 보고하였다.
뇌동맥의 fenestration은 외과적 수술 시에 어려움을 초래할 가능성이 있을 뿐더러 뇌혈관
조영의 판독 과정에 오독을 일으킬 우려가 있다는 점에 그 임상적 의미가 있다. 또한
fenestration 자체에 동맥류가 발생되는 사실은 혈류역학적 stress와 더불어 선천적 요인들
이 뇌동맥류 발생의 한 원인 요소일 가능성을 시사한다.
저자들은 우측 내경동맥 -후교통동맥 동맥류가 파열되어 뇌지주막하 출혈이 초래되었던
한자에서 뇌혈관 조영 및 동맥류 경부 결찰 수술 중에 확인된 동측 Ml 근위부의
fenestration을 관련 문헌 고찰과 함께 보고하고자 한다.

The authors report an anomalous fenestration at the M1 portion of the middle cerebral
artery found incidentally in a patient affected with a ruptured aneurysm at the ipsilateral
internal carotid artery(ICA) and unruptured small aneurysms at the contralateral P1 and
ICA. The anomaly was confirmed both angiographically and intraoperatively. A review
of the literature indicated several reports of this same anomaly, many of which were
associated with aneurysms involving either the proximal end of the fenestration or other
cerebral arteries. In the interpretation of cerebral angiograms and during intracranial
operations, clinical attention should be paid to this rare anomaly.
