Abstract
서 론
환추축성 탈구증의 원인은 외상성, 자발성, 선천성등에 의해 발생하는데 주로 경추외상에
의하며 약 20% 정도에서 선천성 혹은 퇴행성 질환등에 의한다. 정복되지 않는 환추축성 탈
구증은 증상이 평소에 심하지 않았더라도 오랫동안 척수의 압박소견을 나타내어 경미한 외
부 자극에 대해 매우 삼각한 결과를 초래할 수가 있다. 최근 본원에서 척수손상을 동반한
환추축성 탈구환자를 경구감압술에 의한 정복과 후방고정술을 이용하였으나 심한 골다공증
소견으로 인하여 후방골 고정이 실패하여 나시경고정 및 후두 및 경추고정술을 시행하여 양
호한 결과를 얻었기에 보고한다.

We report a case of irreducible atlantaxial dislocation with spinal cord injury. The
patient was a 36-year-old man who suffered from the sudden onset shallow respiration
and severe motor weakness after slip down. We have performed the one-stage
operation, the transoral decompression, reduction and posterior interspinous wiring with
bone fusion. Three weeks after surgery, the reduction state and posterior wiring
procedure were failed. due severe osteoporotic change. So we have performed second
operation. the transarticular facet screw flextion and occipito-cervical fixation with
Steinman pin, from which good postoperative stability and neurological improvement.
