Abstract
서 론
중추신경계의 혈관기형의 일종인 해면상 기형은 간질, 뇌출혈, 종괴효과에 의한 다양한 신
경학적 증상이 발현되기도 하지만 대체적으로 증상을 나타내지 않고 CT나 MRI 검사중 우
연히 발견되는 경우가 많은 질환으로 발생빈도는 0.5%이내이다. 이 병변은 특정 호발 연령
이 없이 모든 연령에서 발견되고 남녀 성비는 비슷하며 뇌동정맥 기형에 비하여 그 자연 경
과가 양호하다고 알려져 있다. 한편 두개강내 종양중 교모제포종은 악성도가 매우 높으며
인구 10만명당 5∼7명의 발생빈도를 보이고, 45∼65세에 호발하며 남자에서 여자보다 발생
비율이 다소 많다. 초기 증상의 발현으로부터 진단 시기까지의 평균기간은 3∼6개월 정도이
다. 본 저자들은 동일 환자에서 해면상 혈관기형을 수술로 완전 적출하여 치료한지 5년후에
과거병소의 인접 부위에 발생한 교모세포종 1례를 경험하여 보고하고자 한다.

The authors report a case of left parietal glioblastoma developed five years after
surgical removal of the frontal cavernous malformation A 35-year-old woman presented
with history of seizure for 13 years and left frontal parasagittal mass on MRI. The
mass was removed uneventfully. and the histopathologic examination revealed a
cavernous malformation. Her seizure disappeared after the surgery. Five years later, the
patients developed new sysptoms of right leg weakness and paresthesia. imaging studies
followed by pathological study revealed left paretal. parasagittal glioblastoma. which was
located posterior to the previous surgical field. Following surgery, she is now on regular
follow up with radioation therapy and chemotherapy. The authors report this rare
occurrence of the glioblastoma following surgical removal of calvernous malformation
with review of psrtinent litsraturss.
