Abstract
서 론
척수 신경근에서 발생하는 신경초종은 대부분 양성으로, 악성 신경초종은 말초신경에서
더 자주 발생하며 척수 및 뇌신경의 신경근에서 발생하는 경우는 드물다.
세포성 신경초종은 신경초종의 한 변형으로서 세포과다, 세포분열 활동도의 증가, 골 조직
침식 등의 소견을 보임으로서 악성 신경초 종양과 유사한 소견을 보이나 생물학적 양상이
양성으로, 드물게 재발하는 경우는 있지만 전이된 경우는 없다. 따라서 세포성 신경초종은
악성 신경초 종양과의 감별이 가장 중요하다.
말초신경에서 발생한 신경초종 중 세포성 신경초종이 차지하는 비율은 약 10%로 알려져
있지만 척수 및 뇌신경근에서 발생한 세포성 신경초종은 보고된 예가 매우 적고 대부분 성
인에서 발생하며 유아에서 발생한 경우 또한 보고된 바가 없다.
본 저자들은 6개월된 유아의 경추부 척수강에서 발생한 세포성 신경초종 1예를 조직학적,
조직면역화학적 방법으로 진단하고 악성 신경초종과의 감별점을 중심으로 보고하고자 한다.

Celluar schwannoma is a very rare benign tumor involving peripheral nerves.
mimicking mailgnant nerve sheath tumor in histological appearance. it is a
clinico-pathological variant of benign schwannoma characterized by a high cellularity.
increased mitotic figures and occasional presence of bone destruction. A 6-month-old
female patient was admitted with progressive quadriparesis. Magnetic resonance imaging
of the cervical spine showed a well-enhancing extramedullary mass on the C4 to C6
area. Total laminectomy was performed on C4-7, and intradural extramedullary tumor
was removed. histologically, the tumor was composed of proliferated spindle cells. Most
of the tumor was composed of Antoni A area with hypercelliar spindle cells arrnaged in
fascicular pattern. The tumor cells showed mild atypism with mitotic figure upto
4/10HPF and diffuse, strong S-100 protein immunostaining. The patient was improved
postopernatively.
