Abstract
서론
수근관 증후군(carpal tunnel syndrome)은 주로 수부의 제 1, 2, 3 수지의 통증과 감각이상
을 초래하는 상지의 정중신경 압박에 의한 질환이다. 40∼60대의 여자에 호발하며 전형적인
증상은 수부의 저림(numbness)과 통증으로서, 밤에 더욱 심한 것이 특징이다. 또한 말초 신
경 압박증상을 보이는 여러 가지 압박신경증(entrapment neuropathy)중 발생빈도가 가장 높
으며, 주로 손목을 많이 사용하는 주부나 농부등에서 잘 발생하고 컴퓨터 단말기 사용 인구
의 증가에 따른 직업병으로서의 발생빈도도 점차 증가하는 추세이다. 또한 수근관 증후군은
신경외과 영역에서 흔한 질환인 경추 신경근병증(cervical radiculopathy)과 비슷한 임상 증
상을 보여 오진되고 진단이 늦어진 경우도 간혹 볼 수 있다.
이에 대한 치료는 소염진통제나 물리치료등의 보존적 요법, 스테로이드 국소 주입법, 수술
적 치료등이 있으며 수술적 치료로는 관혈적 횡수근인대 절개술, retinaculatome같은 특수한
기구를 사용하는 방법, 그리고 내시경을 이용한 방법등이 있다.
최근의 minimally invasive surgery 경향과 발달된 내시경 수술기구 등의 영향으로 내시경
수술에 대한 관심이 점차 높아지고 있다. 본 병원에서는 최근 1년 2개월 동안 10명의 수근
관 증후군 환자에서 Agee Endoscopic Carpal Tunnel Release System을 이용한 횡수근인대
절개술을 시행하여 좋은 결과를 얻었다.

The carpal tunnel syndrome is the most common entrapment neuropathy. Several
treatment modalities are available at present time. Surgical methods vary from open
surgery, alternative surgical method using retinaculatome, and endoscopic surgery. Ten
patients with carpal tunnel syndrome were treated with Agee endoscopic carpal tunnel
release system in our institute from Nov. 1996 to Jan. 1998. The sensory conduction
velocity of the median nerve were delayed or the sensory evoked potential were not
evoked in all patients preoperatively. Preoperative plain radiographs of hands(including
carpal tunnel view) showed normal bony structures of the carpal tunnel in all, one after
a follow up of three months. Although small in number of patients treated, this method
seems to be a better than the other treatment modalities.
