Abstract
결론
본 교실에서 1990년 1월부터 1996년 12월까지 7년간 수술한 134예의 전교통 동맥류 환자에
서 수술전 환자의 상태, 혈관연축, 동맥류의 방향, 크기 및 모양, 수술의 방법과 수술결과 등
여러 인자들의 관계를 비교 분석하여 다음과 같은 결론을 얻었다.
1) 연령 분포는 50대가 가장 많았고, 남녀의 비율은 1 : 1.2로 여자에서 많았다.
2) 동맥류의 방향과 Fisher's grade, 수두증 발생 빈도와는 유의한 상관관계를 보이지 않았
다.
3) 동맥류의 크기, 모양 및 방향은 내원시 Hunt-Hess grade와 유의한 상관관계를 보이지
않았다.
4) 동맥류의 모양은 뇌혈관 조영술상 혈관연축과 유의한 상관관계를 보여 lobulated와 oval
shape에서 혈관연축이 많이 발생했으며, 동맥류의 크기와 방향은 혈관연축과 유의한 상관관
계를 보이지 않았다.
5) 술전 Hunt-Hess grade, 뇌혈관 조영술상 혈관연축 유무, 임시감자 여부는 예후와 유의
한 상관관계를 보여 술전 Hunt-Hess grade가 낮은 경우, 혈관연축이 없는 경우, 임시감자
를 시행한 경우에서 예후가 좋았다.
6) 동맥류의 모양, 크기 및 방향, 윌리스환의 형태학적 기형 유무, 수술시기와 수술방법 등
은 예후와 유의한 상관관계를 보이지 않았다.
7) 수술의 합병증은 혈관연축 및 뇌경색이 18.7%로 가장 많았고 뇌부종, 수두증 등이 있었
으며, 사망률은 3%였다.
#초록#
This study is a retrospective clinical analysis of 134 cases of anterior communicating
artery aneurysms surgically treated in the Department of Neurosurgery, Chungnam
National University Hospital from January 1990 to December 1996.
The results of analysis were summarized as follows ;
1) Peak age incidence was in the sixth decaded and male to female ratio was 1 : 1.2,
showing female predominancy, occurrence of hydrodephalus, and also between the
direction the direction, size and shape of aneurysm and preoperative Hunt-Hess grade.
3) There was statistically significant relationship between the shape of aneurysm and
angiographic vasospasm ; there was high incidence of vasospasm in lobulated and oval
shape of aneurysm. But there was no statistically significant relationship between the
direction, size of aneurysm and angiographic vasospasm.
4) The correlations between outcome and preoperative Hunt-Hess grade, and occurrence
of angiographic vasospasm, and temporary clipping were statistically significant ; the
outcome was good in cases of no vasospasm, temporary clipping and better preoperative
Hunt-Hess grade.
5) The direction, shape and size of aneurysm, existence or not of abnormality in circle
of willis, timing of surgery and operative procedure had no statistically significant
relationship with outcome.
6) Postoperative complications were vasospasm and infarction(18.7%), brain edema,
hydrocephalus, in order of frequency, and the mortality rate was 3%.
