Abstract
서 론
종양내 자발성출혈은 전이성뇌종양 특히 악정흑색종, 융모상피선종, 폐암 등에서 흔히 보
이는 소견인데 원발성 뇌종양중에서는 희돌기 교종, 악성 교종에서 드물게 동반된다고 한다.
일반적인 뇌종양, 특히 뇌수막종은 양성종양으로 임상경과는 서서히 진행되나 종양이 출
혈을 일으킬 때는 뇌졸중과 같이 심한 두통, 의식불명 또는 의식상태의 악화, 반신마비 등의
국소신경학적 장애가 갑자기 발생하여, 뇌혈관질환과 감별이 어려운 경우가 드물지 않다.
전체 뇌종양과 동반된 두개강내 출혈의 빈도는 5% 이내이나 양성종양인 뇌수막종에서는
두개강내출혈이 매우 드문 소견으로 보고되고 있으며, 현재까지 문헌 보고에 의하면 전세계
적으로 80례 미만의 증례가 보고되었고 우리나라에서는 2례가 보고되어 있을 뿐이다.
저자들은 최근 갑작스런 의식장애 또는 심한 두통과 우측하지 부전마비 등 뇌졸중과 유사
한 임상증상으로 내원한 2례와 경도의 두통을 주소로 내원하여 감마나이프 방사선수술 3간
후 종양내 출혈로 인해 갑자기 혼수상태와 양측 하지 부전마비가 발생하여 수술후 출혈을
동반한 뇌수막종으로 확진된 1례를 포함한 수막종과 동반된 두개강내 출혈 3례를 경험하였
기에 문헌고찰과 함께 보고하는 바이다.

Spontaneous intracranial hematoma in Patients with brain tumors is well recognized
However intracranial hematoma associated with meningioma has been rarely reported.
Not infrequently, clinical manifestations of such tumors have been masked by the
associated hematoma, which preclude correct diagnosis of these tumors.
Meningioma-associated hemorrhages have been reported regardless of sex age or
location and histology of the tumor. The mechanisms of such hemorrhage are yet to be
clarified.
During the last two Years we haute experienced three cases of intracranial
hemorrhage associated with meningioma. Two of them occurred acutely without
antecedent symptoms of the meningioma and other one after gamma knife surgery of
meningioma.
We reviewed partinent literature and discussed possible mechanisms of these rare
intratumoral or peritumoral hemorrhages associated with meningiomas.
