결 론
제 1-2경추 불안정증의 수술적 치료로서 후방 고정술이 매우 유용하며 이중 철사를 이용
한 방법은 전통적으로 해오던 방법으로서 수술이 비교적 용이하고 안전한 방법으로서 유용
하다. 한편 나사못을 이용한 경관절 고정술은 전자의 방법보다 정교한 수술 술기가 요구되
나 환자 선택의 폭이 좀 더 광범위하며 우수한 융합율과 수술 후 외고정기 없이 즉각적인
안정화가 가능한 장점이 있다.

The authors analyzed the surgical results of posterior G1-G2 fusion in 18 cases of
atlantoaxial instability Posterior Cl-C2 wiring with clone fusion(11 cases) Cl-C2
transarticular screw fixation(6 cases) and occipitocervical fusion(1 case) have been
performed for unstable odontoid process fractures(10 cases) transverse ligament injury(4
cases), os odontoideum (3 cases). rheumatoid C1-2 instability(1 case). Follow-up
examination was performed in all patients after a mean postoperative duration of 39.9
months. Follow-up x-rays showed successful stabilization in 17cases(11 %) and there
was no major operative complication.
