Abstract
서 론
후대뇌 동맥류의 발생 빈도는 매우 낮은 편으로 전체 뇌동맥류의 약 1% 가량을 차지하
며, 문헌에 따라 0.26∼2.3%의 빈도로 보고되고 있다. 후대뇌 동맥류중 약 80%가 직경 2.5
㎝미만의 낭상 동맥류이고, 방추형 동맥류와 거대 동맥류가 각각 10% 정도를 차지한다. 발
생 부위는 Pl 분절 및 P2 분절에 발생하는 경우가 약 90%로 대부분을 차지하고 있으며, 특
히 P2분절에 발생하는 것이 약56% 정도로 가장 높은 발생 빈도를 보이고 있다. P3 분절 이
하의 원위부 후대뇌 동맥류는 빈도가 더욱 낮아서 후대뇌 동맥류의 약 13%에 불과하다.
후대뇌 동맥류는 다른동맥류와는 달리 동맥류의 발생 부위에 따라 다양한 수술 접근법 및
수술 방법들이 보고되고 있으며, 그중 P3 분절에 발생한 동맥류에 대한 수술적 접근은 측두
하 접근법 (subtemporal approach) 또는 후두 반구간 접근법(occipital interhemispheric
approach) 등이 있다. 본 교실에서는 매우 드문 2예의 P3분절에 발생한 후대뇌 동맥류를 측
두하 접근법을 이용하여 특별한 신경학적 결손 없이 수술 치험하였기에, 문헌고찰과 함께
보고하는 바이다.

Two cases of aneurysms in the P3 segment of the posterior cerebral artery are
described. These were managed by the subtemporal approach One giant aneurysm in a
47-year-old male was treated by clipping the afferent artery, and the other small
saccular aneurysm in a 41-year-old female was treated by clipping the aneurysmal neck.
There were no complications after surgery, and the patients were discharged without
neurological deficit. The operative approaches and procedures are also discussed in
relation to the anatomy of posterior cerebral artery aneurysms.
