Abstract
서 론
뇌혈관 동맥류 발생의 정확한 원인은 아직 논란의 대상이 되고 있다. 하지만 가족력을 가
지고 발생하는 뇌혈관 동맥류는 병태생리를 이해하는데 가치가 있는 단서라 할 수 있을 것
이다
가족력을 가지고 발생하는 뇌혈관 동맥류는 그리 흔하지 않으나 몇몇 저자들의 보고가 있
었다. 아직까지는 정확한 유전의 증거나 유전 양상은 확인되지 않고 있으나, 여러 저자들의
보고를 종합하여 보면 유전적 요소가 중요한 역할을 담당할 것으로 생각된다 특히 다낭종신
(polycystic kidn-ey), 대동맥협착증(coarctation of Aorta), Marfan씨 증후군 Ehlers-Danlos
증후군 및 탄성섬유가황색종(pseud-oxanthoma elasticum)1')과 같은 유전성 질환에서 뇌혈
관 동맥류의 빈도가 높다는 것이 알려져 있다
최근 본 의국에서는 환자의 가족력상 가족성 뇌혈관 동맥류로 의심되는, 모자간에 발생한
후순환계 동맥류를 수술 치험하여 좋은 결과를 얻었기에 문헌 고찰과 함께 보고한다.

Familial intracranial aneurysm is rare clipped two posterior circulation intracranial
aneurysm in mother and son. Mother was 48 years old and presented with severe
hedache and drowsy mentality. A basilar artery bifurcation aneurysm was found on
DSA(digital subtraction angiography) and successfully clipped with trans-sylvian
pterional approach. Her son was 24-years-old and presented with seizure and drowsy
metality. An aneurysmal sac was found on the bifurcation of vertebral artery and
posterior cerebellar artery. We have clipped it successfully with far lateral suboccipital
approach. The authors recommended the use of screening test. such as MRI and high
resolution CT, on the group of patients with the characteristic features of familial
intracranial aneurysm.
