Abstract
서 론
림프구성 뇌하수체염은 드문 질환이나 최근 뇌하수체 기능 저하증의 원인으로 그 빈도가
증가하고 있다. 조직학적으로는 뇌하수체에 림프구(lymphocyte)와 형질세포(plasma cell)의
침착과 섬유화가 특징적이며, 이로 인하여 뇌하수체의 파괴가 일어나는 것으로 알려져 있다.
병인론으로는 자가면역(autoimmunity)이 중요한 요인으로 알려져 있으며, 약 30%에서 다른
종류의 자가면역질환이 동반되는 것으로 보고되고 있다. 주로 분만기주위 (peripartum)의 여
성에게 발생하는 것으로 알려져 왔으나, 남성 또는 임신과 관계없는 여성에서의 발생도 보
고되고 있다. 임상적으로는 뇌하수체기능저하를 동반하는 뇌하수체의 종괴로 발현되는 경우
가 흔한 것으로 알려져 있으나, 림프구성 뇌하수체염의 자연병력(natural history). 방사선학
적 특징 치료방법, 예후 등은 아직 확실히 밝혀져 있지 않다.
저자들은 아직 보고된 예가 없는 전신적 면역질환인 전신성 홍반성 낭창(Systemic Lupus
Erythematosus, SLE)과 동반된 림프구성 뇌하수체염 1 례를 수술치험하고 문헌 고찰과 함
께 보고한다.
Lymphocytic hypophysitis is a rare inflammatory disease if the pituitary gland that is
being recognized increasing as a cause of hypopituitarism. Lymphocytic hypophysitis is
due to autoimmune pituitary destruction and often occurs with other autoimmune
diseases. We report a rare case of lymphocytic hyphophisitis in a 21-year old woman,
who suffered from systemic lupus erythematosus (SLE), presenting with intermittent
headache for 2 months.
Magnetic Resonance Imaging with a contrast medium revealed expanding sellar mass
with hemorrhage. The patient underwent transsphenoidal surgery with a provisional
diagnosis of pituitary adenoma. Histological examination revealed evidence of
lymphocytic hypophysitis. We report a case of lymphocytic hyphocytic hypophysitis in
nonpregnant women who suffered from SLE, an immunologically mediated multisystem
disease.
