Abstract
서론
중대뇌동맥의 동맥류는 모든 두개강내 동맥류의 약 20%를 차지하며 대부분이 중대뇌동맥
의 분지부(bifurcation)에 위치한다. 또한 대뇌반구의 2/3에 해당하는 부분과 기저핵(basal
ganglia) 등이 중대뇌동맥과 그 분지인 렌즈핵 선조체 동맥(lenticulostriate aa.)들에 의해 혈
액을 공급을 받기 때문에 임상증상 또한 다양하다. 다른 뇌동맥류 파열과 비교하여 간질이
발생하는 비율도 높고 혈류의 방향 등의 요인으로 인해 세균성 동맥류(infectious
aneurysm) 등의 발생빈도가 다른 두개강내 혈관 영역보다 높다.

Middle cerebral artery aneurysms account for aporoximately 20 percent of all
intracranial aneurysms. The majority are located at the bifurcation of M1 segment in
sylvian fissure. Incidence of formation of intracerebral hematoma with ruptured
aneurysm and giant and infectious aneurysms are more common than with aneurysm in
other locations. Clinical presentation of the MCA aneurysms are associated with
symptoms of surbarachnoid hemorrhage and mass effect. Symptoms of subarachniod
hemorrhage such as headache are indistinguishable from that associated the SAH from
rupture of an aneurysm in any other location. But mass effect of large aneurysm,
temporal lobe epilepsy and transient ischemic attack occur more frequently with
aneurysm of the MCA. Complications of MCA aneurysm surgery can be divided into
group, one related to subarachiod hemorrhage with aneurysm rupture and the other to
surgical procedure. Vasospasm is major cause of morbiduty and medical complications
such as hypertension, cardiac arrhythmia, pneumonia and GI bleeding are frequently
encountered. Operative complications include occlusion of MCA branch by improperly
placed clip, retraction injury, intraoperative rupture.
