¸ñÀû: ³»°æµ¿¸Æ ºÐÁöºÎ¿¡¼­ ¹ß»ýÇÏ´Â ³úµ¿¸Æ·ù´Â ÈçÇÏÁö ¾Ê¾Æ Àüü ³úµ¿¸Æ·ùÀÇ 3-5% Á¤µµ¸¦ Â÷ÁöÇÏ´Â °ÍÀ¸·Î ¾Ë·ÁÁ® ÀÖÀ¸¸ç, µ¿¸Æ·ù ÁÖÀ§ÀÇ ¿©·¯ õ°ø ºÐÁöµé·Î ÀÎÇØ ¼ö¼ú¿¡ ¾î·Á¿òÀ» ÁÖ°í ÀÖÀ¸³ª, ÇöÀç±îÁö ÀÌµé µ¿¸Æ·ùÀÇ Æ¯Â¡ ¹× ¼ö¼ú ¼ºÀû¿¡ ´ëÇÑ º¸°í´Â

¹æ¹ý: ÃÖ±Ù 8³â°£ º»¿ø¿¡¼­ ³úµ¿¸Æ·ù·Î ¼ö¼úÀ» ½ÃÇà ¹ÞÀº 672·Ê Áß ³»°æµ¿¸Æ ºÐÁöºÎ ³úµ¿¸Æ·ù ȯÀÚ 17·Ê(2.5%)¸¦ ´ë»óÀ¸·Î ÈÄÇâÀûÀÎ ÀÓ»ó ºÐ¼®À» ½ÃÇàÇÏ¿´´Ù. µ¿¸Æ·ùÀÇ ¹æÇâÀº DaPian µî°ú °°ÀÌ »ó¹æ, Àü¹æ, ÈĹæÀ¸·Î ºÐ·ùÇÏ¿´´Ù.
°á°ú: ´Ù¸¥ ³»°æµ¿¸Æ¿¡¼­ ¹ß»ýÇÏ´Â µ¿¸Æ·ù¿Í °°ÀÌ ³²ÀÚ 6·Ê, ¿©ÀÚ 11·Ê·Î ¿©ÀÚ¿¡¼­ È£¹ßÇÏ¿´´Ù. Æò±Õ ¿¬·ÉÀº 44.5¼¼(15¡­68¼¼)À̾ú°í, À̵éÀÇ 41.2%°¡ 40¼¼ ¹Ì¸¸À¸·Î Àüü ³úµ¿¸Æ·ù¿¡¼­ ÀüüÀÇ 17.6%¸¸ÀÌ 40¼¼ ¹Ì¸¸ÀÎ Á¡°ú ´ëÁ¶µÇ¾ú´Ù. µ¿¸Æ·ù´Â ÁÂÃø 11·Ê,
¿ìÃø
6·Ê·Î ÁÂÃø¿¡¼­ È£¹ßÇÏ´Â °æÇâÀ» °üÂûÇÏ¿´´Ù. 17·Ê Áß 9·Ê(53%)¿¡¼­ ´Ù¹ß¼º ³úµ¿¸Æ·ù°¡ °üÂûµÇ¾ú°í, ´ëºÎºÐÀÇ °æ¿ì¿¡¼­ ³»°æµ¿¸Æ ºÐÁöºÎ µ¿¸Æ·ù°¡ ÃâÇ÷ÀÇ ¿øÀÎÀ̾ú´Ù. ¼ö¼ú ¼ºÀûÀº 15·Ê¿¡¼­ ¾çÈ£ÇÑ °á°ú¸¦ º¸¿´À¸¸ç, 5.9%ÀÇ »ç¸Á·üÀ» º¸¿´´Ù.
°á·Ð: ¼ö¼ú Áß Ãµ°ø ºÐÁöÀÇ Æó»ö ¹× ¼ö¼ú Áß µ¿¸Æ·ù ÆÄ¿­ÀÌ ¼ö¼ú ¼ºÀûÀ» ¾ÇÈ­½ÃÅ°´Â °¡Àå Å« ¿øÀÎÀ¸·Î ÆǴܵǸç, µ¿¸Æ·ùÀÇ ¹æÇâÀÌ Àü¹æ ¶Ç´Â ÈĹæÀÏ °æ¿ì ´õ Å« ÁÖÀÇ°¡ ÇÊ¿äÇÒ °ÍÀ¸·Î »ç·áµÈ´Ù.

Objective: Aneurysms at the internal carotid artery(ICA) bifurcation are uncommon and account for about 3 to 5% of all intracranial aneurysms. Moreover, the surgical treatment of these aneurysms has been discussed in detail only by a few
authors.
In this report, we present our experienced cases, and discuss the clinical features and surgical outcome of these aneurysms.
Method: In the last eight years, out of total of 672 patients operated for intracranial aneurysms at our hospital, 17 patients(2.5%) had aneurysms at the ICA bifurcation. The direction of aneurysms were distinguished based upon DaPian's
classification ; superior, anterior, or posterior.
Result: There were female preponderance(M:F=1:1.8) and the left side predominance(11 of 17 cases). Nine patients had multiple aneurysms, and in that cases ICA bifurcation aneurysms were responsible for hemorrhage in most cases. All
aneurysms
were
clipped via the standard pterional approach. Thirteen out of 17 patients had good results and 2 patients fair results ; the surgical outcome was favorable in 88.2% of cases. One who had rebleeding before surgery died.
Conclusion: It is presumed that the perforator occlusion and intraoperative aneurysmal rupture were the main factors aggravating the surgical outcome. In cases with anteriorly- or posteriorly-projecting aneurysm, more careful cautions
would
be
necessary.
