Abstract
서론
신세뇨관성 산증은 대표적인 신장의 세뇨관의 기능이상으로 사구체에서 여과된 중탄산염
의 재흡수 부족 혹은 수소이온 배설의 불충분 등으로 인하여 산 염기상태의 불균형, 즉, 산
증이 일어나는 상태이다. 그 중 원위부 신세뇨관성 산증은 원위세뇨관의 산성화의 선택적인
장애가 주요 병리기전으로 저칼륨성, 고염소성 대사성산증을 특징으로 하며 Jaeger 등에 의
해 자발적으로 호전된 갑상선 기능항진증과 임파구성 갑상선염이 동반된 증례가 보고된 바
있다. Yoshiiwa 등은 Hashimoto씨 갑상선염과 원위부 세뇨관성 산증, Sj gren씨 증후군이
empty sella syndrome과 동반된 예를 발표하였고 Nakamura 등은 이들 질환이 만성간염과
합병된 경우를 보고하였다. Caron 등이 류마티스 관절염과 연관된 다발성 자가면역증후군을
연구하는 과정에서 Sj gren씨 증후군과 그레이브스씨 질환의 연관 가능성을 제시하였고
Perez 등이 Sj gren씨 증후군과 합병된 갑상선 질환에 대해 연구한 보고는 다수 있으나 아
직 신세뇨관성 산증 및 Sj gren씨 증후군과 그레이브스씨 질환이 동반되었던 경우는 문헌
보고가 된 적이 없었다.
저자들은 최근 그레이브스씨 질환 및 Sj gren씨 증후군과 동반된 원위부 신세뇨관성 산
증 1예를 경험하고 치유하였기에 문헌고찰과 함께 보고하고자 한다.

Classic distal renal tubular acidosis(type 1 RTA) is a hypokalemic, hyperchloremic
metabolic acidosis due to selective defect in distal acidification. Type 1 RTA may be
associated with genetic(usually autosomal dominant) or autoimmune disease, drugs and
toxin and various tubulointerstitial diseases.
This case report describes a 27 year-old female patient with distal RTA, Sj gren's
syndrome and Graves' disease the first case. She had been suffered from muscle
weakness and peripheral numbness and was found to have low serum
potassium(1.6mEq/L). She was revealed to have 1) distal RTA by
NaH-CO3 loading test 2) Sj gren's syndrome by Schirmer's test and
palate biopsy and 3) Graves' disease by exophthalmometry and autoantibody test,
thyroid function test. This is the first case as far as I search that report the association
of distal RTA, Sj gren's syndrome, and Graves' disease caused by autoimmune
mechanism.
