Abstract
서론
우리 나라를 비롯한 아시아의 여러 나라에서는 잉어(Cyprinus carpio)의 담낭 생식이 시
력 증진 및 건강증진에 효과가 있다고 예로부터 알려져 왔으며, 각종 한방요법 및 민간요법
에서 강장제 혹은 보약으로 최근까지도 그 사용빈도가 흔하다.
1976년 대만의 Chen 등은 초어(Ctenopharygodon idellus)의 담낭을 생식한 후 급성 신부
전과 간기능장애를 나타낸 6예의 증례에 대한 고찰을 한 바 있으며, 국내에서는 1985년 이
등, 1987년 남 등과 1990년 박 등이 잉어 담낭을 복용한 후 나타난 급성신부전과 급성독성
간염에 대한 보고가 있었다. 잉어의 종류, 크기, 섭취량, 섭취 방법 및 개인차에 따라 정도의
차이는 있지만 잉어의 담낭 복용 후 나타나는 임상양상으로 복통, 오심, 구토, 설사 등의 위
장관계의 증상은 공통적인 초기증상이며, 발열, 오한, 두통, 현훈, 경련, 혼수, 혈뇨, 무뇨, 급
성신부전, 황달, 급성독성간염 등으로 다양하게 나타날 수 있다고 보고되고 있다.
지금까지 잉어과의 담낭생식으로 유발된 급성 신부전 및 급성간염의 증례는 주로 잉어와
초어, 은어 등에 의한 경우가 대부분이었고, 향어에 의한 예는 1987년 남 등이 1예의 보고가
있었다. 저자들은 향어(이스라엘 잉어; Cyprinus carpio nudus)의 담낭 생식 후 병발된 급성
간염 및 급성신부전 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

Since 1976, several cases of acute renal failure and acute hepatitis after ingestion of
raw carp bile have been reported. We experienced 1 case of acute renal failure and
hepatitis after ingestion of the raw fish(Cyprinus carpio nudus) gall bladder.
The patient ate raw fish gall bladder to improve the health. The size of the fish he
ate was over 1m and that of the gall bladder was about an egg size. The clinical
manifestations were characterized by dizziness, headache, diffuse abdominal pain, both
flank pain, nausea, vomiting and diarrhea. These symptoms were followed by gross
hematuria, oliguria and jaundice. Elevation of blood urea nitrogen, creatinine,
transaminases lasted for about 2 weeks. Biopsy of the kidney was performed 13 days
after ingestion of raw fish gall bladder. The kidney pathology showed recovery phase of
acute tubular.
The patient recovered with conservative treatment.
