Abstract
목적: 선천 이중눈물점은 대게 편측에 나타나며, 덧눈물점이 내측에 존재하여 정상눈물점의 모양과는 구별되는 미성숙한 슬릿 형태로 존재하는 것이 일반적이다. 기존의 보고와는 다르게 외측에 덧눈물점이 존재하는 양상의 선천 이중눈물점 환자를 경험하였기에 이를 보고하고자 한다.
증례요약: 기저 질환 및 특별한 안과적 과거력 없는 39세 남자 환자가 2주 전부터 시작된 우안 눈물 흘림을 주소로 내원하였다. 세극등검사에서 우하측에 크기와 형태가 같은 눈물점 2개가 관찰되었으며 이외의 다른 눈물점들은 정상적인 단일 눈물점이었다. 탐침검사 및 코눈물관관류검사에서 우측의 부분적인 코눈물관 협착소견을 보여 실리콘관삽입술을 계획하였다. 우하측 내측눈물점을 주눈물점으로 판단하여 이 눈물점에 실리콘관삽입술을 시행하였으며, 술 후 증상은 호전되었다.
결론: 한국에서 아직까지 보고되지 않은 드문 선천 이중눈물점 증례로서, 기존의 문헌 보고와는 다르게 외측눈물점이 덧눈물점인 점이 특이하다고 생각되는 바 이를 보고하고자 한다. 덧눈물점이 외측에 존재할 수 있으므로 정확한 코눈물관관류검사 및 탐침검사를 통해 덧눈물점과 주눈물점을 구별하는 것이 필요하다.
Purpose: Congenital double puncta are usually unilateral, and the accessory punctum exists on the medial side in a slit configuration that is distinct from the shape of the normal punctum. We report a case of an unusual case of double lacrimal puncta which the lateral, rather than the medial, punctum was judged to be the accessory punctum.
Case summary: A 39-year-old male patient with no underlying disease and no ophthalmologic history visited our clinic with right eye epiphora of 2 weeks duration. On slit lamp examination, double puncta were observed in the right lower eyelid and the remaining puncta were normal. On lacrimal syringing test and dacryocystography were performed and revealed incomplete obstruction with partial narrowing of the nasolacrimal duct. Silicone tube intubation was performed through the right lower medial punctum and symptoms improved postoperatively.
Conclusions: The present case is an unusual case of double lacrimal puncta which has not been reported in Korea. Unlike the previous literature, the lateral, rather than the medial, punctum was judged to be the accessory punctum. Because accessory punctm can be present on the lateral side, it is necessary to distinguish between the accessory punctm and the main punctum through the accurate dacryocystography and lacrimal syringing test for the treatment of the patient.
