Abstract

¸ñÀû: ¼ö¼ú ºÎÀ§ ¼Òµ¶¿¡ »ç¿ëµÈ °í³óµµ ¿¡Åº¿Ã¿¡ ÀÇÇØ ¹ß»ýÇÑ °¢¸·È­»ó 1¿¹¸¦ º¸°íÇÏ°íÀÚ ÇÑ´Ù.

Áõ·Ê¿ä¾à: 56¼¼ ³²ÀÚ È¯ÀÚ°¡ ¿ìÃø ¾È¿Í°ñÀýÁ¤º¹¼úÀ» ¹Þ´ø Áß, ¾È¸éºÎ ¼Òµ¶À» À§ÇØ »ç¿ëÇÑ 2% Çí½ÃŸ³î ¿ë¾×(2% Chlorhexidine, 72% Ethanol È¥ÇÕ¾×)ÀÌ Á¾ȿ¡ Èê·¯ µé¾î°£ ä·Î ¼ö¼úÀ» ¸¶Ä¡°í ³ª¼­ ¹ß»ýÇÑ ÅëÁõÀ» ÁÖ¼Ò·Î ³»¿øÇÏ¿´´Ù. ¼¼±ØµîÇö¹Ì°æ°Ë»ç»ó ÁÂ¾È À±ºÎÁÖÀ§ °á¸·ÃæÇ÷, °¢¸·ÇÏÃø»óÇÇ °á¼Õ°ú ±âÁú ÀüüÀÇ ½ÉÇÑ È¥Å¹ ¹× ºÎÁ¾ÀÌ °üÂûµÇ¾ú´Ù. °í³óµµ ¿¡Åº¿Ã¿¡ ÀÇÇÑ ÁÂ¾È °¢¸·È­ÇÐÈ­»óÀ¸·Î Áø´ÜÇÏ°í, Ç×»ýÁ¦ ¹× ±¹¼Ò ½ºÅ×·ÎÀ̵å Á¡¾È Ä¡·á¸¦ ½ÃÀÛÇÏ¿´´Ù. 1ÁÖ ÈÄ °¢¸·»óÇÇ°á¼ÕÀº È£ÀüµÇ¾úÀ¸³ª, Ä¡·á 2°³¿ù° ³»ÇǼ¼Æ÷ ±â´ÉÀúÇÏ¿Í ÇÔ²² ¹Ýº¹°¢¸·¹Ì¶õÀÌ Àç¹ßµÇ¾î, ¾Õ±âÁúõÀÚ¼ú°ú Ä¡·á·¹ÀÌÀú°¢¸·ÀýÁ¦¼úÀ» ½ÃÇàÇÏ¿´´Ù. °á·Ð: 2% Çí½ÃŸ³î ¿ë¾×Àº ¼ö¼ú Àü ÇǺμҵ¶À» À§ÇØ »ç¿ëµÇ°í ÀÖÀ¸³ª °í³óµµÀÇ ¿¡Åº¿ÃÀ» ÇÔÀ¯ÇÏ°í ÀÖ´Ù´Â »ç½ÇÀº °£°úÇϱ⠽±´Ù. °í³óµµÀÇ ¿¡Åº¿Ã ¿ë¾×ÀÌ ¾È±¸¿¡ ³ëÃâµÇ¸é °¢¸·³»ÇǼ¼Æ÷ ºÎÀü ¹× °¢°á¸· À±ºÎ¼¼Æ÷ÀÇ ¼Õ»óÀ» Æ÷ÇÔÇÑ ½ÉÇÑ °¢¸·¼Õ»óÀ» ÃÊ·¡ÇÒ ¼ö Àֱ⿡ ÁÖÀÇÇÏ¿©¾ß ÇÑ´Ù.

Purpose: To report a case of chemical injury of the cornea caused by high-dose ethanol during orbital wall fracture repair.

Case summary: A 56-year-old male presented with pain after blowout fracture repair surgery. During the surgery, 2% hexethanol solution (2% chlorhexidine and 72% ethanol mixture), which was used for disinfection of the face, flowed into the left eye. Conjunctival injection in the left limbus, a large corneal epithelial defect, and severe stromal edema were subsequently observed. The patient was treated with topical antibiotics, steroids, and autologous serum eye drops. After 1 week, the corneal epithelial defect was improved, but at the second month of therapy, recurrent corneal erosion with deterioration of the endothelial cell function occurred. Anterior stromal puncture and laser keratectomy were performed. The corneal epithelial defect and erosion improved, but the endothelial cell density was severely decreased.

Conclusions: The 2% hexethanol solution is usually used for preoperative skin disinfection, but it contains a high concentration of ethanol. The surgeon should be aware that high concentrations of ethanol may result in severe corneal damage, including corneal endothelial dysfunction and limbal cell deficiency.
