Abstract
목적: 시신경유두 드루젠과 이에 동반된 맥락막신생혈관에서 유리체강 내 항혈관내피세포성장인자(베바시주맙)주입술 시행 후 망막출혈의 감소 및 시력호전을 보인 증례를 경험하여 이를 보고하고자 한다.
증례요약: 기저질환이 없는 53세 여자가 갑작스럽게 발생한 좌안의 시력저하를 주소로 내원하였다. 최대교정시력 우안 1.0, 좌안 0.8, 안압은 양안 15 mmHg로 측정되었고 세극등검사상 전안부에서 특이 소견은 관찰되지 않았다. 안저검사상 좌안에 망막하출혈 및 장액성 망막박리 소견이 관찰되었으며, 형광안저혈관조영 및 인도시아닌그린 형광안저혈관조영검사상 시신경유두 근처 부위에서 과형광부위를 확인하였다. 양안의 시신경유두 드루젠과 좌안에 동반된 맥락막신생혈관 진단하에 유리체강 내 베바시주맙주입술을 시행하였고 3주 후 망막출혈은 호전되는 경과를 보였다.
결론: 시신경유두 드루젠에서 동반된 맥락막신생혈관에서 유리체강 내 항혈관내피세포성장인자 주입술이 효과적인 치료로 고려될 수 있음을 보고하는 바이다.
Purpose: To describe a patient with optic nerve head drusen who showed improved retinal hemorrhage and visual acuity following intravitreal anti-vascular endothelial growth factor (bevacizumab) injection.
Case summary: A 53-year-old woman with no underlying disease presented with sudden vision loss in her left eye. Her best-corrected visual acuity was 1.0 in the right eye and 0.8 in the left eye; the intraocular pressure was 15 mmHg in both eyes. Anterior segment examination revealed no abnormal findings. Fundus examination showed subretinal hemorrhage and serous retinal detachment in the left eye. Fluorescein angiography and indocyanine green fundus examination revealed hyperfluorescence near the optic disc. The patient was diagnosed with choroidal neovascularization of the left eye secondary to optic nerve head drusen; intravitreal bevacizumab injection was then performed. Three weeks later, the patient showed improved retinal hemorrhage.
Conclusions: Intravitreal anti-vascular endothelial growth factor injection may be an effective treatment for choroidal neovascularization associated with optic nerve head drusen.
