Abstract

¸ñÀû: ´«¹°±æÆó¼â ȯÀÚ¿¡¼­ ´«¹°±æ¹Ì¼¼³»½Ã°æ»ó °üÂûµÇ´Â Æó¼âÀÇ À§Ä¡¿Í Á¤µµ°¡ ½Ç¸®ÄÜ°ü»ðÀÔ¼úÀÇ ¼º°ø·ü¿¡ ¹ÌÄ¡´Â ¿µÇâÀ» Æò°¡ÇÏ°íÀÚ ÇÏ¿´´Ù.

´ë»ó°ú ¹æ¹ý: 2018³â 8¿ùºÎÅÍ 2019³â 10¿ù±îÁö ´«¹°±æ¹Ì¼¼³»½Ã°æÇÏ ½Ç¸®ÄÜ°ü»ðÀÔ¼úÀ» ½ÃÇà ¹ÞÀº 120¾È 74¸íÀÇ È¯ÀÚµéÀ» ´ë»óÀ¸·Î ÈÄÇâÀû Â÷Æ® ¸®ºä¸¦ ½ÃÇàÇÏ¿´´Ù. ¼ö¼ú ¼º°ø·ü°ú ȯÀÚÀÇ ÀÓ»ó¾ç»ó, ¼ö¼ú Àü °ü·ù°Ë»çÀÇ °á°ú, ´«¹°±æ¹Ì¼¼³»½Ã°æ °üÂû¾ç»óÀ» °¢°¢ ºñ±³ ºÐ¼®ÇÏ¿´À¸¸ç, ´«¹°±æ¹Ì¼¼³»½Ã°æ»ó °üÂûµÇ´Â Æó¼âÀÇ À§Ä¡¿Í Á¤µµ¸¦ ºÐ·ùÇÏ¿´´Ù.

°á°ú: ´«¹°±æ¹Ì¼¼³»½Ã°æÇÏ ½Ç¸®ÄÜ°ü»ðÀÔ¼úÀ» ½ÃÇà ¹ÞÀº 120¾È Áß ¼ö¼ú ¼º°øÀº 101¾È(84.2%)À̾ú´Ù. ¹Ì¼¼³»½Ã°æ»ó ´«¹°¼Ò°üÇùÂø 41¾È(34.2%), ´«¹°¼Ò°üÆó¼â 35¾È(29.2%), ÄÚ´«¹°°üÇùÂø 24¾È(20.0%), ÄÚ´«¹°°üÆó¼â 28¾È(23.3%)ÀÌ °üÂûµÇ¾ú´Ù. ´«¹°¼Ò°ü°ú ÄÚ´«¹°°üÀÇÆó¼âÀÇ À§Ä¡¿¡ µû¶ó ¼º°ø·ü¿¡´Â Â÷ÀÌ°¡ ¾ø¾úÀ¸¸ç, Æó¼â Á¤µµ¿¡ µû¶ó ¼º°ø·ü¿¡ ¿µÇâÀ» ¹ÌÄ¡´Â °ÍÀ» È®ÀÎÇÏ¿´´Ù(p=0.041, p=0.018).

°á·Ð: ´«¹°±æ¹Ì¼¼³»½Ã°æ ¼Ò°ßÀ» ¹ÙÅÁÀ¸·Î ÄÚ´«¹°°üÆó¼â¸¦ ºÐ·ùÇÒ ¼ö ÀÖÀ¸¸ç, Æó¼âÀÇ À§Ä¡´Â ½Ç¸®ÄÜ°ü»ðÀÔ¼úÀÇ ¼º°ø·ü°ú °ü°è°¡ ¾ø¾ú°í, ´«¹°±æÆó¼âÀÇ Á¤µµ´Â ½Ç¸®ÄÜ°ü»ðÀÔ¼úÀÇ ¼º°ø·ü¿¡ Åë°èÀûÀ¸·Î À¯ÀÇÇÑ ¿µÇâÀ» ¹ÌħÀ» ¾Ë ¼ö ÀÖ¾ú´Ù.

Purpose: In patients with lacrimal passage obstruction, the location and severity of the obstruction observed in lacrimal endoscopy were evaluated with respect to the success rate of silicone tube intubation.

Methods: We conducted a retrospective chart review of 120 eyes of 74 patients who underwent lacrimal endoscopy (RUIDO fiberscope; Fibertech Co., Tokyo, Japan)-assisted silicone tube intubation from August 2018 to October 2019. The analysis included clinical characteristics, irrigation test results, lacrimal endoscopic findings, and analyzed factors associated with surgical success. The location and severity of the obstruction observed by lacrimal endoscopy were classified.

Results: Of the 120 eyes that received lacrimal endoscopy-assisted silicone tube intubation, 101 eyes (84.2%) were successful in surgery. Canalicular stenosis in 41 eyes (34.2%), canalicular obstruction in 35 eyes (29.2%), nasolacrimal duct stenosis in 24 eyes (20.0%), and nasolacrimal obstruction in 28 eyes (23.3%) were observed in lacrimal endoscopy. Regardless of the location of the obstruction of the canaliculus and nasolacrimal duct, it was observed that surgical success was affected by the severity of the obstruction (p = 0.041 and p = 0.018, respectively).

Conclusions: The severity of the lacrimal passage obstruction observed during lacrimal endoscopy was shown to have a statistically significant effect on the success rate of lacrimal endoscopy-assisted silicone tube intubation. However, the location of the obstruction in the lacrimal passage did not have a statistically significant effect on the success rate.
