Abstract

¸ñÀû: ¼­ºÎ °æ³² Áö¿ª 3Â÷ ÀÇ·á±â°ü¿¡¼­ Áø´ÜÇÑ °¨¿°¾È³»¿° ȯÀÚµéÀÇ ¿øÀαÕ, ÀÓ»ó¾ç»ó ¹× ½Ã·Â¿¹Èĸ¦ ¾Ë¾Æº¸°íÀÚ ÇÏ¿´´Ù.

´ë»ó°ú ¹æ¹ý: 2010³â 1¿ùºÎÅÍ 2019³â 6¿ù±îÁö °æ»ó´ëÇб³º´¿ø¿¡¼­ °¨¿°¾È³»¿°À¸·Î Áø´ÜµÈ 93¸í(98¾È)À» ´ë»óÀ¸·Î ÈÄÇâÀûÀ¸·Î Àǹ«±â·ÏÀ» ºÐ¼®ÇÏ¿´À¸¸ç ¸ðµç ȯÀÚ¿¡¼­ Àü¹æ°ú À¯¸®Ã¼ °ËüÀÇ ¹è¾ç°Ë»ç¸¦ ½ÃÇàÇÏ¿´°í ¹è¾çµÈ ±ÕÀÇ Á¾·ù, Ä¡·á ¹æ¹ý, ½Ã·Â¿¹ÈÄ µîÀ» ºÐ¼®ÇÏ¿´´Ù.

°á°ú: ´ë»ó ȯÀÚ 98¾È¿¡¼­ ±ÕÀÌ ¹è¾çµÈ °æ¿ì´Â 67¾È(68.4%)À̾úÀ¸¸ç °¡Àå ÈçÇÑ °¨¿°¾È³»¿°ÀÇ ¿øÀÎÀº ¹é³»Àå¼ö¼ú ÈÄ ¹ß»ýÇÑ ¾È³»¿°À̾ú°í. °¡Àå ÈçÇÑ ¿øÀαÕÀº ±×¶÷¾ç¼º±ÕÀÎ Staphylococcus epidermidis (16.3%)À̾ú´Ù. ÃÖÁ¾ ÃÖ´ë±³Á¤½Ã·ÂÀ» 1.0 logMARÀ» ±âÁØÀ¸·ÎºÐ¼®ÇÑ °á°ú ÃÊÁø ÃÖ´ë±³Á¤½Ã·ÂÀÌ ¾ÈÀü¼öÁö ÀÌ»óÀÎ °æ¿ì, À¯¸®Ã¼ÀýÁ¦¼úÀ» ½ÃÇàÇÑ °æ¿ì°¡ Åë°èÀûÀ¸·Î ÀǹÌÀÖ´Â ¿¹Èĸ¦ º¸¿´´Ù(p<0.001, p=0.021).

°á·Ð: °¨¿°¾È³»¿° Áß ¿ÜÀξȳ»¿°Àº ¹é³»Àå¼ö¼ú ÈÄ ¸¹Àº ¹ß»ý ºóµµ¸¦ º¸ÀÌ¸ç ±×¶÷¾ç¼º±ÕÀÎ Staphylococcus epidermidis°¡ °¡Àå ÈçÇϸç ÁÁÀº ½Ã·Â¿¹Èĸ¦ º¸¿©ÁÖ¾ú´Ù. ³»Àξȳ»¿°Àº °£³ó¾çÀÌ °¡Àå ÈçÇÑ °¨¿°º´¼ÒÀÌ°í ±×¶÷À½¼º±ÕÀÎ Klebsiella pneumoniae°¡ ÈçÇÏ°í ³ª»Û ½Ã·Â¿¹Èĸ¦ º¸¿©ÁÖ¾ú´Ù.

Purpose: To investigate the bacterial culture, clinical characteristics, and prognosis associated with infectious endophthalmitis in western Gyeongsangnam-do.

Methods: This study was a retrospective review of the medical records of 98 eyes that presented with infectious endophthalmitis from 2010 to 2019. All patients were analyzed for the type of causative bacteria, treatment method, and the visual prognosis.

Results: The bacterial cultures (n = 98 eyes) revealed bacterial growth in 67 eyes (68.4%). The most commonly isolated causative microorganism was Gram-positive Staphylococcus epidermidis (14.3%). As a result of analyzing the best corrected visual acuity (VA) based on 1.0 (logMAR), baseline VA was worse than counting fingers, and whether pars plana vitrectomy was undertaken showed a difference in the final visual acuity (p < 0.001, p = 0.021).

Conclusions: Exogenous endophthalmitis occurs frequently after cataract surgery, and S. epidermidis is the most
common causative agent, but a good visual prognosis resulted. Liver abscess was a common extraocular source of endogenous endophthalmitis and Klebsiella pneumonia was the most common causative agent for a poor visual prognosis.
