Abstract
결론
저자들은 1989년 12월부터 1993년 2월까지 NCI Working Formulation에 따라 중등도 또
는 고도의 림프종으로 진단 받은 30례의 후흉선 T세포 림프종 환자와 37례의 미만성 B세포
림프종 환자를 대상으로 그 임상상과 치료에 대한 반응 및 예후 등을 비교하여 다음과 같은
결과를 얻었다. 후흉선 T세포 림프종은 미만성 B세포 림프종에 비해 진단 당시 진행된 병
기에 전신증상을 잘 동반하였으며, 고도의 조직아형을 가지는 환자가 많았고, 골수나 피부,
비강 및 부비동의 림프절외 조직침범을 잘하는 특성을 갖았다 3년 생존률이 후흉선 T세포
림프종군이 46.9%,미만성 B세포 림프종군이 67%로 두 군간에 유의한 차이를 보였는데
(p=0.0489), 후흉선 T세포 림프종 환자 중 진행된 병기에 발견된 활동능력이 떨어진 환자들
의 나쁜 치료성적이 그 원인으로 사료된다. 그러나, 후흉선 T세포 림프종 환자 중 적극적
인 항암치료가 가능하였던 군에서는 미만성 B세포 림프종과 유사한 완전관해율 및 완전관
해군에서 무병생존률을 보여, 후흉선 T세포 림프종은 단일 질환군이 아닌 이질적인 예후를
지니는 질환군의 집합으로 생각된다.

Purpose : Peripheral T-cell lymphoma(PTCL) derived from mature T cells forms
morphologically diverse group of non-Hodgkin's lymphomas and the clinicopathologic
features remain to be debated. In order to elucidate the specific characteristics of PTCL,
comparison with a group of diffuse B-cell lymphomas(DBCL) was done.
Materials and Methods : Between Dec. 1989 and Feb. 1993, clinical data of 67 cases of
intermediate or high grade NHL identified as T-cell or B-cell origin by immuno-
phenotyping was reviewed.
Results : There were 30 cases of PTCL and 37 cases of DBCL. PTCL had more
advanced stage and B symptoms at diagnosis. Frequent sites of extranodal involvement
were bone marrow, nasal cavity/paranasal sinus, and skin in PTCL and gastrointestinal
tract in DBCL. Based on NCI Working Formulation, 40% of PTCL and 14% of DBCL
were high grade. Patients with DBCL had a better 3-year overall survival rate(67% vs
47%), however, there was no difference in complete remission rate and disease-free
survival rate between two groups with intensive treatment.4 subgroup of PTCL patients
who had died earlier was found to have more advanced stage and poor performance
status.
Conclusion : Although patients with PTCL had worse survival in advanced stage, the
outcome of patients with PTCL who received intensive treatment was comparable to
that of DBCL.
