Abstract
서론
전신성 홍반성 루푸스(systemic lupus erythematosus, SLE)는 면역조절의 이상을 특징으
로 하는 자가면역성 결체조직 질환의 일종으로 림프종 같은 악성 종양의 발생위험이 정상인
에 비하여 높다고 알려져 있다. 또한, SLE는 림프절 종대가 진단 당시에는 약 7% 및 질환
의 경과 시에는 약 12%가 발견된다고 하는데, 이 경우 다른 질환 특히 암세포의 전이로 인
한 림프절 종대와 감별하기가 쉽지 않다. 외국의 경우 SLE 환자에게서 림프종, 다발성 골수
종, 자궁경부암 등 악성종양의 동반발생이 보고된 바 있었으나, 우리 나라에서는 이에 관한
보고를 찾아보기가 힘든 실정이다 이에 저자 등은 SLE로 진단 받고 2년간 부신피질호르몬
요법을 시행하던 중, 우측 경부 림프절 종대가 발견되어 본 질환의 악화로 생각하고 부신피
질호르몬 제제의 용량을 증가하였으나 림프절 종대가 진행되어 조직 생검을 한 결과 편평
상피 세포암으로 진단되어 치료한 1예를 경험하였기에 보고하는 바이다.

Systemic lupus erythematosus(SLE) is one of the connective diseases characterized by
altered immunity and has a high incidence of cancers, such as malignant lymphoma,
leukemia, breast cancer and cervical cancer. Lymphadenopathy is one of the clinical
findings of SLE. But it is also seen in cancer metastasis. Therefore, if the SLE patient
has lymphadenopathies from cancer metastasis it is very difficult to make a differential
diagnosis between the manifestation of SLE and that of cancer metastasis. In Korea,
there is no report on SLE associated with lymphadenopathies from cancer metastasis.
This is the first report of a case of SLE with a cervical lympadenopathy that comes
from the metastasis of head and neck squamous cell carcinoma.
