서론
고립성 형질세포종(Solitary Plasmacytoma: SP)인 고립성 골형질세포종(Solitary Bone
Plasmacytoma; SBP)과 골수외 형질세포종(Extramedullary Plasmacytoma; EMP)은 전체
형질세포종에서 약 7%를 차지하는 드문 경우이다. 그 중 고립성 골형질세포종(SBP)이 두
개내에서 발생하는 비율은 0∼18%정도로 보고되고 있으나 현재까지 국내에서의 보고는 없
었다. 저자들은 좌측 안구돌출을 주소로 내원한 68세 남자 환자에서 개두술을 통해 중두개
와에서 발생하며 안와를 침습한 고립성 골형질세포종을 진단하였고 21개월간 추적관찰을 통
해 다발성 골수종으로의 이행이 확인되어 문헌고찰과 함께 보고한다.
Solitary plasmacytoma consists of solitary bone plasmacytoma(SBP) and
extramedullary plasmacytoma. Its frequency is about 7% among the total plasma cell
neoplasm. Solitary bone plasmacytoma of the cranial cavity occurs in 0∼18% of patients
with SBP. We experienced a case of SBP originated from the middle cranial fossa in a
68-year-old man. After surgical removal of the mass, biopsy specimen revealed
plasmacytoma, kappa type. There was no evidence of systemic involvement. Additional
radiotherapy was performed. Twenty-one months after the diagnosis of SBP, pathologic
fracture of the left humerus happened. Biopsy specimen of the operation revealed same
diagnosis. At that time, Bence Jones proteinuria was detected in immuno-electrophorisis
of urine and simple bone X-rays showed multiple osteolytic lesions. A case of SBP of
the orbit and middle cranial fossa in a 68-year-old man is presented and the literature
is reviewed.
