임신중 발견된 급성 골수성 백혈병 1예

Abstract
서론
임신중 급성 백혈병으로 진단 받고 이에 대한 치료를 받은 환자에 대한 보고는 많지 않
다.백혈병은 임신 전후반에 걸쳐 발생할 수 있는데, 임신중 항암화학요법이 산모나 태아에
미치는 영향은 명확히 밝혀져 있지 않다. 임신중에 투여할 수 있는 항암제도 제한되어 있으
며, 더욱이 산모의 백혈병이 관해에 이르더라도 장기적인 백혈병 치료와 태아에 대한 항암
제의 위험 등을 고려하여 치료의 강도를 결정해야 할 것으로 여겨진다
임신과 동반된 종양의 치료에 대한 국내의 보고로는 비호지킨 림프종으로 치료 경력이 있
는 임산부에서 임신중기에 조직전환된 비호지킨성 림프종으로 진단 받고 항암요법과 함께
임신을 유지하여 만삭의 정상여아를 질식분만한 예가 있었으며 임신중 급성백혈병으로 진단
받고 항암화학요법을 받아 출산한 증례가 드물게 보고되어왔다. 저자들은 임신후기에 급성
골수성 백혈병으로 진단 받고 제태 기간 중에 관해유도요법을 시행하였는바, 관해에 도달한
산모와 관해유도중 조기분만으로 출생하였지만 일과성 범혈구 감소증외에는 정상소견을 보
인 신생아를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

The incidence of acute leukemia in pregnancy is rare. The treatment of acute
leukemia during pregnancy is complicated and therapeutic options must be made with
each individual patient. Complete remission can now be achieved in 60 to 70% of
previously untreated adults with acute myelogenous leukemia(AML). Antileukemic
chemotherapy can be administered safely during the second and third trimesters.
Cytarabine(ara-C) and anthracycline has not been associated with birth defect. When a
pregnant woman presents with acute leukemia, chemotherapy should be recommended as
vigorously as in the non-pregnant woman.
We reported a case of AML during pregnancy. The patient recieved induction
chemotherapy with tora-C and idarubicin. The baby was delivered at 33 weeks of
gestation and had transient neutropenia. The mother received consolidation chemotherapy
after achievement of complete remission.
