Abstract
서론
악성 식도 협착의 고식적 치료 방법의 하나인 자가 팽창성 금속 스텐트는 최근 반복적 확
장술, 우회 수술, 방사선 요법, 약물 요법, 레이저 요법, 인공 구조물 삽입 등의 다른 고식적
치료 방법에 비해 연하 장애, 식도-기도간 누공 등에 비교적 쉽고 빠르며 효과적인 치료 방
법으로 보고되고 있다.
식도용 자가 팽창성 금속 스텐트는 구조, 재질에 따라 다양한 종류가 있다 자가 팽창성
금속 스텐트는 기존의 플라스틱 튜브 인공 삽입물에서 가장 문제시되었던 합병증인 식도 천
공과 위치 이동율을 훨씬 낮췄다고 보고되고 있다.
자가 팽창성 금속 스텐트 위치 이동은 스텐트의 후기 합병증 중 흔히 예견되어 온 합병증
이지만 위치 이동에 의해 유문 폐쇄 유발 예는 현재까지 보고 된 바가 없고 그 임상적 중요
성도 다른 합병증에 비해 유병율과 사망율에 영향이 크고 스텐트 개발이 위치 이동을 줄이
기 위한 여러 시도와 함께 이어져 왔던 점등으로 미루어 앞으로 많은 연구와 사례 보고가
이어져야 될 것이다.
저자들은 식도암 환자에서 연하 장애의 고식적 치료의 방법으로 자가 팽창성 금속 스텐트
의 하나인 송 스텐트를 삽입한 후 위치 이동이 되어 유문 폐쇄를 일으킨 1예를 경험하였기
에 문헌 고찰과 함께 보고하는 바이다.

Placement of the self-expandable metallic stents for palliative treatment of malignant
esophagogastric strictures has been thought to be easy, fast and effective method than
conventional methods(bypass procedures, radiation therapy, laser treatment, esophageal
intubation, etc.). The expandable metallic stent tubes were found to overcome some of
the limitations of nonexpandable conventional tubes. Their implantation is better tolerated
and safer than that of nonexpandable tubes, because the risks of migration and
perforation are lower,
On our knowledge, there has been no report of pyloric obstruction after this metallic
stent insertion.
We hereby report a case of pyloric obstruction caused by a migrated self-expandable
metallic stent for palliative treatment of malignant esophageal stricture.
