식도암의 방사선치료성적

Abstract
연구목적 : 식도암의 치료성적을 향상시키고자 근치적 방사선치료와 수술후 방사선치료를
시행한 식도암 환자의 생존율, 예후인자 및 재발양상에 대하여 분석을 시도하였다.
대상 및 방법 : 1989년 9월부터 1995년 12월까지 충남대학교병원 치료방사선과에서 식도
암으로 근치적 및 수술후 방사선치료를 받은 98명 줄 3,000 cGy이상 치료받은 87명을 대상
으로 후향적 분석을 시행하였다. 연령분포는 28-80세 (중앙값 61세)였고 병리조직소견상 편
평상피세포암이 80예, 선암이 1예, 선편평상피세포암이 3예, 소세포암이 3예였다. 병소의 위
치별 분포는 경부식도 2예, 상흉부식도 7예, 중흉부식도 44예, 하흉부식도 26예, 그리고 중복
병소가 6예였다. 병기별 분포 (1992년 AJCC)는 Ⅰ기 3예, ⅡA기 22예, ⅡB기 9예, Ⅲ기 45
예, Ⅳ기 8예였으며 병소의 크기에 따라 5 can 이하가 56예, 5 cm초과가 31예였다. 치료 목
적에따라 근치적 방사선치료군이 57예였고 수술후 방사선치료군이 30예였다. 근치적 방사선
치료군 중 6예는 방사선치료 단독으로, 51예는 항암화학요법을 병행하였으며 12예에서는 외
부방사선치료 후 고선량률 근접방사선치료를 시행하였다. 총조사선량은 근치적 방사선치료
시 3,060∼7,440cGy (중앙값 5,940 cGy)였으며 수술후 방사선 치료시 4,500∼6,480 cGy (중
앙값 5,040 cGy)였다. 추적기간은 2개월 - 69개월 (중앙값 10개월)였다.
결 과 : 전체 2년, 5년 생존율 및 중앙생존기간은 각각 29.7%, 11.2%, 14개월였다. 병기별
로 병기 Ⅱ, Ⅲ, Ⅳ의 2년 생존율은 각각 39.3%, 23.6%, 0% (p=0.0148)였다. 병소의 크기에
따른 2년 생존율은 5 can이하와 5 can 초과시 각각 38.3%와 9.3% (p=0.001)였고, 치료목적
에 따라 근치적 방사선치료군과 수술후 방사선치료군의 2년 생존율은 각각 19.6%와 46.7%
(p=0.0081)였다. 수술후 방사선치료군에서 임파절 전이 유무에 따른 2년 생존율에는 통계적
인 차이가 없었다. 근치적 방사선치료후 객관적 치료효과를 확인할 수 있었던 39예에서 완
전관해가 20예 (51.3%), 부분관해는 16예 (41%),무반응 3예 (7.7%)였으며, 완전관해를 보였
던 군과 수술후 방사선치료군의 2년 생존율은 각각 33.1%와 46.7%(p=0.5828)로 유의한 차이
가 없었다. 예후인자로서는 병기 (p=0.0148)와 병소크기 (p=0.001)가 통계적으로 유의하였다.
재발양상을 추적할 수 있었던 근치적 방사선치료군 49예와 수술후 방사선치료군 23예에서
국소재발, 원격전이, 그리고 국소재발 및 원격전이 동시재발이 각각 6예 (12.2%), 7예
(14.3%), 2예 (4.1%)와 3예 (13%), 8예 (34.8%), 2예 (8.7%)였다.
결 론 : 근치적 방사선치료를 받고 완전관해를 보였던 환자군과 수술후 방사선치료군 간
의 생존율에 유의한 차이는 없었으며, 식도암의 예후인자로서 병기와 병소 크기가 중요하였
다. 진행된 병기의 식도암 환자의 국소제어와 생존율을 높이기 위해서는 보다 적절한 항암
화학방사선 치료방법과 고선량률 근접 방사선치료에 대한 연구가 필요하리라 사료된다.

INTRODUCTION
Esophageal cancer primarily affects older men and ranks fifth (3.3%) in Korean male
cancer incidence. Cigarette smoking and alcoholic beverage drinking are the major
causes of esophageal cancer in western countries, and account for the fourfold excess in
male compared with female. The overwhelming majority of esophageal cancers present
with locally advanced or metastatic disease. Despite the major efforts in recent decades,
the prognosis for patients with carcinoma of the esophagus remain poor. Major reviews
of reported surgical and radiotherapeutic studies revealed an average 5-year survival of
4∼6%. The low cure rate is primarily caused by the difficulty in achieving local control
with radiation therapy or surgery. Other major causes include the high incidence of
lymph node metastases and distant metastases.
In recent years, numerous institutions and cooperative groups have reported the
results of combined modality chemotherapy and radiation therapy in the management of
patients with esophageal cancer, either as curative treatment or as pre- or postoperative
therapy. Reported 2-year actuarial survival rates have ranged from 20∼54% in the
series where in chemoradiotherapy comprised curative therapy. For chemoradiotherapy as
planned preoperative treatment, 2-year actuarial survival rates of 15∼66% have been
obtained.
In this study, we analyzed retrospectively the results of 87 patients treated with
curative radiotherapy and postoperative radiotherapy including prognostic factors
influencing the longterm survival and the patterns of failure.
