Abstract
서론
위에 생긴 악성 림프종은 위에 원발성으로 발생할 수도 있지만, 그 보다는 전신질환의 일
환으로서 위를 침범하는 경우가 더 많다. 따라서 원발성 위림프종의 진단에 필요한 기준은,
환자의 증상이 모두 또는 주로 소화기 계통의 증상들이어야 하고, 병변이 위에 국한되어 있
거나 위에서 기원한 것이라야 한다 위림프종의 발생 빈도는 서양에서는 위의 악성 종양중
2∼5%로 보고되고 있으며 위에 발생하는 모든 육종 중에는 약 60%를 차지한다. 그러나 우
리나라에서는 위선암의 빈도가 높아 위의 악성종양중 차지하는 빈도는 1% 미만이다. 이러
한 희귀성 때문에 이에 대한 보고가 많지 않은 실정이다.
또한 최근들어 원발성 위림프종 중에서 Mucosa-associated lymphoid tissue lymphoma
(MALT lymphoma)의 개념이 도입되었고 이 MALT lymphoma는 다른 위 림프종에 비하여
예후가 좋은 것으로 나타나고 있어 과거에 원발성 위 림프종으로 진단 받은 환자들에 대한
병리학적 재검토가 필요하게 되었다. 또 림프종의 일반적인 staging system인 Ann-Arbor
staging system을 그대로 위림프종에 적용했을 경우, 종양의 침윤 깊이가 고려되지 않기 때
문에 부적절 하다는 주장들이 제기되고 있다.
따라서, 저자들은, 외과적 절제술을 시행한 위림프종의 임상적, 병리학적 특성을 밝히고,
이들의 예후 인자로서의 의미를 평가해보고, 또한 원발성 위림프종 중 MALT lymphoma가
차지하는 비율과 이들의 예후를 알아보고자 본 연구를 시작하였다.
Purpose : The incidence of primary gastric lymphoma is a relatively rare and is 2∼
5% of all gastric malignant lesion. Because of its rarity, there are few report in Korea.
Therefore, to elucidate the clinicopathological characteristics and prognosis of primary
gastric lymphoma and to investigate MALT(Mucosa-associated lymphoid tissue)
lymphoma, we studied in primary gastric lymphoma. Materials 6r Methods: we evaluated
retrospectively primary gastric lymphoma patients who underwent gastric resection from
Jan. 1990 to Dec. 1994 in KCCH.
Results : There were 20 patients with primary gastric lymphoma and the incidence is
0.54% of all gastric malignant lesion. Abdominal pain, primarily epigastric, was the most
common presenting complaint and the most common location was the distal third of the
stomach. The overall 5-year survival rate was 71% Factor significantly influencing
5-year survival rate was the serosal invasion. Age, gender, tumor size, histologic grade,
nodal status and Ann-Arbor staging system did not influence survival.
Conclusion : So, we suggest that the depth of invasion should be included in staging
system of gastric lymphoma because Ann Arbor staging system might be inadequate
when applied to primary gastric lymphoma. Also, we suggest that the multicenter study
should be performed to elucidate the characteristics of Korean gastric lymphoma because
of its low incidence.
