대장암 환자 혈청을 이용한 표피 성장 인자 수용체(EGFr) 및 변헝 성장 인자 알파(TGF-α)의 동시 측정

Abstract
연구 목적 : EGFr및 TGF-α는 대장 점막의 정상 상피 세포에서 표현될 뿐만 아니라 대
장암 환자에서도 매우 다양하게 과표현되는 것으로 알려져 있다. 본 연구는 대장암 환자의
혈청을 이용한 EGFr 및 TCF-α의 동시 측정과 정상 대조군에서의 EGFr 및 TGF-α 측정
과 비교 분석하여 임상적 유용성을 밝히고자 하였다.
대상 및 방법 : 모두 42예의 대장암 환자와 건강한 정상 성인 20예가 대조군으로 사용되
었다. 두 군간에 성별과 연령은 유사하였다. EGFr및 TGF-α 측정은 EGFr
AssayTM system 및 TGF-α AssayTM system을 사용하여
ELISA방법으로 측정하였다.
결 과 : 대장암 환자의 혈청 EGFr은 413.8±91.4 fm/ml(mean±SD; range, 252 to 641
fm/ml)로 대조군의 296.0±53.5 fm/ml보다 유의하게 높았다(p=0.0001). 대조군의 평균+2SD
를 상한선으로 하였을 때의 EGFr 전체 양성률은 52%(22/42)였다. 병기별 EGFr농도는 병기
A 447.0 fm/ml (n=1), 병기B 428.7±89.2 fm/ml (n=11), 병기C 413.9±112.3 fm/ml (n=19),
병기D 395.6±53.6 fm/ml (n=11)로 병기별 유의성은 없었다. 대장암 환자의 혈청 TGF-α는
92.6±43.9 pg/ml(mean±SD; range, 36 to 231 pg/ml)로 대조군의 56.2± 12.0 pg/ml보다 유
의하게 높았다(p=0.001). 대조군의 평균+ESD를 상한선으로 하였을 때의 TGF-α 전체 양성
률은 50%(21/42)였다. 병기별 TGF-α 농도는 병기A 84.1 pg/ml (n=1), 병기B 112.8±60.2
pg/ml (n=11), 병기C 84.7±34.8 pg/ml (n=19), 병기D 78.3 ± 33.1 (n=11)로 병기 별 유의성
은 없었다. 대장암 환자에서 혈청 EGFr 증가함에 따라 동시에 TGF-α도 증가하여 두 인자
사이에는 유의한 상관 관계가 있었다(r2=0.23, p<0.05). 혈청 CEA양성률은
47%(20/42)로 EGFr과 TGF-α의 양성율과 유사하였으나 이를 동시 측정하였을 때 양성률
의 상승을 보였다.
결 론 : 본 연구는 EGFr및 TGF-α의 자가분비 고리(autocrine loop)를 대장암 환자 혈청
을 이용한 실험으로 확인하였다. 또한, 비록 병기별 유의성은 없었으나 대장암 환자 혈청내
EGFr 및 TGF-α는 대조군에 비해 현저히 높은 것으로 나타나 향후 더 많은 환자를 대상
으로 임상적 유용성에 대한 연구가 필요할 것으로 본다.
INTRODUCTION
Epidermal growth factor receptor (EGFr) is a membrane bound glycoprotein expressed
on the surface of epithelial cells. EGFr is activated when its ligand binds to the
extracellular domain resulting in phosphorylation of the receptor's intracellular domain by
a family of growth factors. For mammalian cells, in addition to epidermal growth factor
(EGF), these include transforming growth factor-α (TGF-α), amphiregulin, and
heparin-binding EGF.
EGFrs are expressed at varying levels on normal bowel epithelium and malignant
colon epithelium. Collectively, 25∼77% of tumors express detectable levels of EGFr by
Western blotting with anti EGFr-antibodies or by immunohistochemistry. Approximately
50∼90% of colon tumors express TGF-α and a subpopulation of tumors contains
higher levels of TGF-α protein than that of matched normal colonic mucosa. TGF-α
is structurally and functionally related to EGF and acts exculsively through EGFr. Also,
increased EGFr expression is often associated with increased production of TGF-α by
the same tumor cells in a subset of colon tumor. These results for coexpression suggest
that a potential autocrine loop involving TGF-α may be operative in vivo for some of
these tumors.
The present study investigated the serum EGFr levels, serum TGF-α levels, and the
well known biomarker carcinoembryonic antigen (CEA) in patients with colorectal cancer
and control group. We evaluated the clinical significance of the results in association
with tumor stage, histology, and other parameters. Finally, we investigated the
relationship between serum EGFr levels and serum TGF-α levels. We used the enzyme
linked immunosorbent assay (ELISA) system to quantitate EGFr and TGF-α in the
sera of colorectal cancer patients. Serum EGFr extracellular domain (EGFr ECD) levels
were measured for the detection of EGFr.
