Abstract
서론
암의 발생은 여러 인자들이 작용하고 또한 여러 단계를 거쳐서 일어나며, 최근 분자생물학
의 발전으로 세포의 악성변형(malignant transformation)이 바이러스감염, 암억제유전자의
이상, 암유전자의 활성화 등 다양한 유전학적 변이가 복합적으로 관여하여 발생한다고 밝혀
졌다. 세포고사란 유전자 발현의 변화로 발생하는 세포자신의 파괴로 다세포기관의 성장, 분
화 및 사멸의 생체균형을 유지하는데 꼭 필요한 정상 생리적 과정이며, 종양조직 내에서는
종양세포의 성장속도를 조절하고 방사선 치료나 항암제 치료에 대한 종양세포의 반응정도에
관여하는 중요한 과정이다. 세포고사는 c-myc, p53, pRB, c-fos, bc12, bax와 같은 유전자나
HPV E6, E7 암단백, adenovirus EIA, EIB 암단백에 의하여 조절되는 것으로 알려져 있으
나, 후두평편세포암에서 종양세포의 T병기, 림프절 전이 유무, 재발여부 및 치료결과에 따른
세포고사 정도에 대한 연구는 미흡한 실정이다. 따라서 저자들은 후두편평세포암에서 세포
고 사체를 검출하여 종양의 임상병리소견 및 치료결과에 따른 세포고사지수를 산출하여 후
두편평세포암에서 세포고사지수의 임상적 의의와 후두편평세포암의 생물학적 특성을 이해하
는데 도움을 얻고자 하였다
Purpose : The purpose of this study was to analyze the relationship between apoptotic
index and clinicopathological findings in laryngeal squamous cell carcinomas.
Materials and Methods : Foully-one cases of laryngeal squamous cell carcinomas were
analyzed for the detection of apoptotic bodies by in situ hybridization.
Results : The average apoptotic index was 9.38±2.89. The apoptotic indices were 11.26
±1.86, 8.17±2.55, and 4.32±2.41 in well differentiated, moderately differentiated and
poorly differentiated carcinoma, respectively. The histopathological differences and
apoptotic index were statistically significant(p<0.05). But there was no relationship
between apoptotic index and T stage, nodal status, recurrence and treatment results.
Conclusion : The apoptotic index was only related to cellular differentiation. Also these
results suggest that the numbers of apoptotic bodies in the tumor tissues can make it
possible to presume indirectly the malignant potentiality of the tumor and will help us
to understand the biologic behavior of head and neck cancer.
