Abstract
서론
침습성 아스페르길루스증(invasive aspergillosis)은 면역기능저하 환자에서 발생하는 감염
증으로, 최근 장기이식술 및 악성종양에 대한 고용량 항암화학요법이 시행됨에 따라 빈도가
증가하고 있다. 발병 인자로는 지속적 인 호중구감소증, 면역억제제의 사용, 숙주의 면역기
능저하, 광범위 항생제의 사용 등이 있으며, 그 중 지속적인 호중구감소증이 가장 중요한 인
자로 알려져 있다. 기관지 원주(bronchial cast)는 기관지내 분비물의 증가 및 배출장애를 가
진 폐 질환에 흔히 동반되며, 원인질환으로는 폐렴, 기관지확장증, 만성 기관지염, 알러지 기
관지폐 아스페르길루스증, 점액성점착증(mucoviscidosis) 등이 알려져 있다. 국내에서 기관
지 원주를 형성한 침습성 아스페르길루스증은 보고된 바가 없으며, 특히 침습성 아스페르길
루스증이 주로 급성 백혈병 환자에서 항암화학요법 후 호중구감소증 상태에서 흔히 발병하
고, 호중구감소증으로 인한 염증반응이 미약하기 때문에 기관지 원주를 형성한 예는 극히
드문 것으로 생각된다. 저자들은 급성 골수성 백혈병 환자에서 항암화학요법 실시 도중 거
대 기관지 원주를 형성한 침습성 아스페르길루스증을 경험하였기에 문헌 고찰과 함께 보고
하는 바이다.
Invasive aspergillosis is a life-threatening infectious disease in immunocompromised
patients. Prolonged neutropenia is the most important predisposing factor. Bronchial casts
are generally associated with pneumonia, bronchiectasis, asthma, chronic bronchitis,
allergic bronchopulmonary aspergillosis and mucoviscidosis. We experienced. a case of
invasive aspergillosis characterized by the formation of large bronchial cast in patient
with acute myelogenous leukemia(AML) during induction chemotherapy The patient
expectorated a large bronchial cast without airway obstruction. Aspergillus was disclosed
in intraluminal surface of bronchial cast. The patient's conditions improved after
amphotericin B therapy and expectoration of a large bronchial cast.
