Abstract
서론
유방암은 현재 우리 나라 여성 암중 위암, 자궁암에 이어 3번째로 많은 악성 종양이다. 최
근 우리 나라도 사회경제적 발달로 국민대중들의 건강에 대한 인지도가 증가하고 방송매체
들의 적극적인 계몽활동에 힘입어 유방암에 대한 관심이 증가하고 있고 또한 식생활이 점차
서구화 되어가면서 발생빈도도 과거보다 증가하는 추세에 있다. 본원의 통계에서도 80년대
초반에는 1년에 약 50여명이 유방암으로 수술을 받았으나 최근에는 1년에 약 200여명으로
증가하는 추세이다. 외국의 여러 보고들에 의하면 젊은 나이에 발생한 유방암의 경우, 나이
든 여성에서 발생한 경우보다 예후가 좋다, 나쁘다, 또는 비슷하다고 하여 논란이 있는 실정
이다. 이에 저자들은 비교적 젊은 나이에 유방암이 많이 발생하는 우리 나라에서 과연 젊은
여성에서 발생한 유방암의 예후가 좀 더 나이든 연령층의 유방암보다 좋은지, 나쁜지를 알
아보고 젊은 나이 자체가 단독으로 예후에 영향을 미치는 인자가 될 수 있는 지를 알아보기
위하여 본 연구를 시행하였다.
Purpose : There is still much controversy about the prognosis of breast cancer
developed in young women compared with old women. We performed this study to
evaluate the prognosis of the breast cancer in young women.
Material and Methods : From 1985 to 1994, 1189 women received operations for breast
cancers at Severance Hospital. The study group included patients less than 36 years old
who had unilateral, invasive and primary operable breast cancers (N=158). The control
groups included patients between 36 and 50 years old (N=518) and those between 51
and 65 years old (N=269) who had the same conditions as the study group. The 5-year
survival and 5-year disease-free survival rate for three groups were compared using
Kaplan-Meier method and Log-rank method. To evaluate the age as an independent
prognostic factor in premenopausal women Cox's proportional hazard model was used.
Result : The overall 5-year survival rate and 5-year disease-free survival rate of the
study group were significantly lower than those of control groups (p<0.05). There was
no significant difference in 5-year survival and 5-year disease free survival between the
two control groups. The Cox's propotional hazard model analysis revealed that the stage
is the most important prognostic factor and the age was also an independent prognostic
factor.
Conclusion : The prognosis of breast cancer less than 36 year old was poorer than
that of 36∼51 year old and 51∼65 year old, suggesting that the age may be an
independent prognostic factor in premenopausal women. More aggressive adjuvant
treatment is required for breast cancer patients less than 36 year old of age.
