Abstract
서론
위암은 우리 나라에서 발생 및 사망 빈도가 가장 높은 악성종양이다. 현재까지 알려진 위
암의 예후인자 중 가장 의의있는 것은 암세포의 위벽침윤도, 림프절 전이 및 원격전이에 의
해 결정되는 TNM 병기이다. 그러므로 위암의 수술후 치료 방침을 정하거나 예후를 예측하
는 데에 TNM 병기가 사용되어 왔으나 동일한 병기의 환자에서도 재발 및 장기 생존 여부
가 다양하기 때문에 기존의 TNM분류방법을 보완할 수 있는 새로운 예후 예측인자를 찾고
자 하는 노력이 시도되고 있다.
최근 분자생물학의 발달에 힘입어 암의 발생은 축적된 유전자의 변화의 결과라는 것이 정
설로 받아들여짐에 따라 암유전자, 성장인자 및 그의 수용체 혹은 암억제 유전자를 암표식
자나 예후인자로 이용하려는 많은 보고가 있었다. 면역조직화학적 분석법은 비교적 간단하
면서도 그 결과가 분자생물학적 방법과 거의 일치하여 다수의 검체를 검사하는 데 시간과
노력을 절약할 수 있다는 장점이 있다. 위암의 경우에도 면역조직화학적 방법을 이용한 많
은 보고가 있었으나 연구 결과가 일치하지 않는 경우가 적지 않다. 그 이유로는 면역조직화
학적 분석의 결과가 조직 처리 방법, 사용한 항체, 염색 방법, 판독, 대상 환자의 수, 병기,
추적기간 등에 따라 달라질 수 있기 때문이다.
이에 저자들은 진행성 위암 중 근치적 수술로 완치를 기대할 수 있으나 임상에서 적지 않
은 재발을 경험하는 UICC 병기분류법(4)에 따른 Ⅱ기 및 ⅢA기 환자에 국한하여 암억제
유전자인 p53, 암유전자인 c-erbB-2, 암의 전이와 관련된 nm23 및 암의 증식 정도를 나타
내는 Ki-67단백질에 대한 면역조직화학 염색을 시행하였다. 또한 이들 단백의 발현과 환자
의 임상병리학적 소견 및 예후와의 상관성을 환자의 생존율 조사를 통하여 알아보고 예후
인자로서의 유용성을 알아보고자 하였다.
Purpose : We investigated the prognostic impacts of p53, c-erbB-2, nm23 and Ki-67
expression in patients with stage Ⅱ and ⅢA gastric carcinoma who underwent curative
(R0) resections.
Materials and Methods : 261 paraffin-embedded gastric carcinoma tissues (stage Ⅱ,
121 ; stage ⅢA, 135) were stained with the monoclonal antibodies, p53, c-erbB-2, nm23
and Ki-67 using the labelled streptovidin biotin method. The positivity was determined
by two pathologists who were kept blind for the patients' outcome.
Results : The overexpression was seen in 51.7% for p53, 11.9% for c-erbB-2, and
70.1% for nm23. The mean Ki-67 labelling index was 25.5±16.7. The rates of
overexpression between the stage Ⅱ and stage ⅢA were not significantly different in
all these molecules. Overexpression of p53 was more likely to be associated with old
age and lymph node metastasis. Overexpression of c-erbB-2 was more likely to be
associated with Borrmann type Ⅰ, Ⅱ and well-differentiated tumor However, nm23 was
more frequently expressed in patients with older age and well-differentiated tumor. In
survival analysis, the overexpressions of p53 and Ki-67 were significantly associated
with poor prognosis of the patients (p<0.01), but c-erbB-2 and nm23 were not related
to the patients' outcome. In a multivariate analysis of prognostic factors, only the lymph
node metastasis was an independent prognostic factor.
Conclusion : Although the values did not reach statistical significance in a multivariate
analysis, the overexpression of p53 and Ki-67 tended to have adverse effects on the
prognosis of patients with gastric cancer.
