Abstract
목 적 : 저자들은 뇌성마비가 발생된 미숙아에서 제대혈 사이토카인 농도를 비교분석하여 뇌성마비 발생과 연관이 있는지 알아보고자 본 연구를 시행하였다.
방 법 : 2001년 9월부터 2003년 6월까지 부산 성분도병원 신생아 집중 치료실에 입원한 미숙아 중 제대혈 사이토카인 검사와 뇌초음파 검사를 실시하였던 환아 60명을 대상으로 후향적인 연구를 통하여 뇌성마비가 초래된 대상군(n=4)과 정상 대조군(n=56)에서 IL-6, IL-1β, TNF-α 농도를 ELISA kit로 측정해 분석하였다. 뇌실주변부 백질연화증이 있는 집단에서 뇌성마비가 초래된 양성군(n=4)과 음성군(n=8)도 비교분석 하였다. 통계적 분석은 통계 패키지 SPSS(version 10.0 windows)를 이용해 wilcoxon test 및 χ2-test로 처리하였고, 통계적 유의수준은 P<0.05로 하였다.
결 과 : 1) 총 60례 중 뇌성마비군이 4례(6.7%), 대조군은 56례(93.3 %)였으며, 평균 재태기간, 출생체중, Apgar 점수, IL-1β, TNF-α, 조기양막파수, 호흡곤란증후군, 패혈증 및 폐렴 등은 두 군간에 유의한 차이는 없었고, 제대혈 사이토카인 중 IL-6만 대상군의 519.63±336.31 pg/mL로 대조군의 82.57±156.06 pg/ mL에 비해 통계학적으로 유의한 차이를 보였다(P<0.05). 2) 분만가사, 패혈증, 조기양막파수, 미숙아 호흡곤란 증후군 및 폐렴 등을 배제한 뇌성마비군에서 IL-6가 507.75±259.15 pg/mL로 대조군 67.73±108.76 pg/mL에 비해 통계학적으로 유의한 차이를 보였고(P<0.05), IL-1β와 TNF-α는 유의성은 없으나 높게 나왔다. 3) 뇌실주변부 백질연화증 집단에서 뇌성마비가 온 양성군이 4례, 음성군은 8례로 양성군의 제대혈 사이토카인 농도 IL-1β와 TNF-α는 음성군에 비해 통계적으로 유의하지 않으나 증가되었고, IL-6는 유의하게 증가되었다.
결 론 : 친염증성 사이토카인(proinflammatory cytokine) 농도와 뇌성마비 발생과 연관을 볼 때 IL-1β, TNF-α는 대조군과 유의한 차이가 없었고 IL-6는 통계학적으로 유의한 증가를 관찰할 수 있었다. IL-6의 농도는 뇌신경세포 손상 정도와 관련성이 높아 뇌성마비의 조기진단에 유용하게 사용할 수 있을 것으로 사료되며 더 많은 연구가 필요할 것이다.
Purpose : This study was performed to determine the relation between cord blood plasma cytokines responses and development of cerebral palsies in premature infants.
Methods : Interleukin-1β(IL-1β), Interleukin-6(IL-6), Tumor necrosis factor-α(TNF-α) were measured using ELISA kits in premature infants(n=60) who were admitted to St. Benedict Hospital from September 2001 to June 2003. Retrospective study was done by review of medical records.
Results : Cord blood levels of IL-1β, IL-6 and TNF-α in the cerebral palsy group were higher, especially IL-6 but were not significant, compared with the control group. Cord blood levels of IL-6 in the cerebral palsy without asphyxia, sepsis, PROM, RDS and pneumonia were significantly different, compared with the control group.
Conclusion : Cord blood levels of IL-1β, IL-6 and TNF-α in the cerebral palsy group were increased, but concentrations of IL-6 increased significantly. Cord blood levels of IL-6 may be a useful value to predict the development of cerebral palsy, because they are related to IL-6 rather than to TNF-α & IL-1β in this study.
