Abstract

¸ñ Àû : ÀúÀÚµéÀº ³ú¼º¸¶ºñ°¡ ¹ß»ýµÈ ¹Ì¼÷¾Æ¿¡¼­ Á¦´ëÇ÷ »çÀÌÅäÄ«ÀÎ ³óµµ¸¦ ºñ±³ºÐ¼®ÇÏ¿© ³ú¼º¸¶ºñ ¹ß»ý°ú ¿¬°üÀÌ ÀÖ´ÂÁö ¾Ë¾Æº¸°íÀÚ º» ¿¬±¸¸¦ ½ÃÇàÇÏ¿´´Ù.

¹æ ¹ý : 2001³â 9¿ùºÎÅÍ 2003³â 6¿ù±îÁö ºÎ»ê ¼ººÐµµº´¿ø ½Å»ý¾Æ ÁýÁß Ä¡·á½Ç¿¡ ÀÔ¿øÇÑ ¹Ì¼÷¾Æ Áß Á¦´ëÇ÷ »çÀÌÅäÄ«ÀÎ °Ë»ç¿Í ³úÃÊÀ½ÆÄ °Ë»ç¸¦ ½Ç½ÃÇÏ¿´´ø ȯ¾Æ 60¸íÀ» ´ë»óÀ¸·Î ÈÄÇâÀûÀÎ ¿¬±¸¸¦ ÅëÇÏ¿© ³ú¼º¸¶ºñ°¡ ÃÊ·¡µÈ ´ë»ó±º(n=4)°ú Á¤»ó ´ëÁ¶±º(n=56)¿¡¼­ IL-6, IL-1¥â, TNF-¥á ³óµµ¸¦ ELISA kit·Î ÃøÁ¤ÇØ ºÐ¼®ÇÏ¿´´Ù. ³ú½ÇÁÖº¯ºÎ ¹éÁú¿¬È­ÁõÀÌ ÀÖ´Â Áý´Ü¿¡¼­ ³ú¼º¸¶ºñ°¡ ÃÊ·¡µÈ ¾ç¼º±º(n=4)°ú À½¼º±º(n=8)µµ ºñ±³ºÐ¼® ÇÏ¿´´Ù. Åë°èÀû ºÐ¼®Àº Åë°è ÆÐÅ°Áö SPSS(version 10.0 windows)¸¦ ÀÌ¿ëÇØ wilcoxon test ¹× ¥ö2-test·Î ó¸®ÇÏ¿´°í, Åë°èÀû À¯ÀǼöÁØÀº P<0.05·Î ÇÏ¿´´Ù.

°á °ú : 1) ÃÑ 60·Ê Áß ³ú¼º¸¶ºñ±ºÀÌ 4·Ê(6.7%), ´ëÁ¶±ºÀº 56·Ê(93.3 %)¿´À¸¸ç, Æò±Õ ÀçűⰣ, Ãâ»ýüÁß, Apgar Á¡¼ö, IL-1¥â, TNF-¥á, Á¶±â¾ç¸·Æļö, È£Èí°ï¶õÁõÈıº, ÆÐÇ÷Áõ ¹× Æó·Å µîÀº µÎ ±º°£¿¡ À¯ÀÇÇÑ Â÷ÀÌ´Â ¾ø¾ú°í, Á¦´ëÇ÷ »çÀÌÅäÄ«ÀÎ Áß IL-6¸¸ ´ë»ó±ºÀÇ 519.63¡¾336.31 pg/mL·Î ´ëÁ¶±ºÀÇ 82.57¡¾156.06 pg/ mL¿¡ ºñÇØ Åë°èÇÐÀûÀ¸·Î À¯ÀÇÇÑ Â÷À̸¦ º¸¿´´Ù(P<0.05). 2) ºÐ¸¸°¡»ç, ÆÐÇ÷Áõ, Á¶±â¾ç¸·Æļö, ¹Ì¼÷¾Æ È£Èí°ï¶õ ÁõÈıº ¹× Æó·Å µîÀ» ¹èÁ¦ÇÑ ³ú¼º¸¶ºñ±º¿¡¼­ IL-6°¡ 507.75¡¾259.15 pg/mL·Î ´ëÁ¶±º 67.73¡¾108.76 pg/mL¿¡ ºñÇØ Åë°èÇÐÀûÀ¸·Î À¯ÀÇÇÑ Â÷À̸¦ º¸¿´°í(P<0.05), IL-1¥â¿Í TNF-¥á´Â À¯ÀǼºÀº ¾øÀ¸³ª ³ô°Ô ³ª¿Ô´Ù. 3) ³ú½ÇÁÖº¯ºÎ ¹éÁú¿¬È­Áõ Áý´Ü¿¡¼­ ³ú¼º¸¶ºñ°¡ ¿Â ¾ç¼º±ºÀÌ 4·Ê, À½¼º±ºÀº 8·Ê·Î ¾ç¼º±ºÀÇ Á¦´ëÇ÷ »çÀÌÅäÄ«ÀÎ ³óµµ IL-1¥â¿Í TNF-¥á´Â À½¼º±º¿¡ ºñÇØ Åë°èÀûÀ¸·Î À¯ÀÇÇÏÁö ¾ÊÀ¸³ª Áõ°¡µÇ¾ú°í, IL-6´Â À¯ÀÇÇÏ°Ô Áõ°¡µÇ¾ú´Ù.

°á ·Ð : Ä£¿°Áõ¼º »çÀÌÅäÄ«ÀÎ(proinflammatory cytokine) ³óµµ¿Í ³ú¼º¸¶ºñ ¹ß»ý°ú ¿¬°üÀ» º¼ ¶§ IL-1¥â, TNF-¥á´Â ´ëÁ¶±º°ú À¯ÀÇÇÑ Â÷ÀÌ°¡ ¾ø¾ú°í IL-6´Â Åë°èÇÐÀûÀ¸·Î À¯ÀÇÇÑ Áõ°¡¸¦ °üÂûÇÒ ¼ö ÀÖ¾ú´Ù. IL-6ÀÇ ³óµµ´Â ³ú½Å°æ¼¼Æ÷ ¼Õ»ó Á¤µµ¿Í °ü·Ã¼ºÀÌ ³ô¾Æ ³ú¼º¸¶ºñÀÇ Á¶±âÁø´Ü¿¡ À¯¿ëÇÏ°Ô »ç¿ëÇÒ ¼ö ÀÖÀ» °ÍÀ¸·Î »ç·áµÇ¸ç ´õ ¸¹Àº ¿¬±¸°¡ ÇÊ¿äÇÒ °ÍÀÌ´Ù.

Purpose : This study was performed to determine the relation between cord blood plasma cytokines responses and development of cerebral palsies in premature infants.

Methods : Interleukin-1¥â(IL-1¥â), Interleukin-6(IL-6), Tumor necrosis factor-¥á(TNF-¥á) were measured using ELISA kits in premature infants(n=60) who were admitted to St. Benedict Hospital from September 2001 to June 2003. Retrospective study was done by review of medical records.

Results : Cord blood levels of IL-1¥â, IL-6 and TNF-¥á in the cerebral palsy group were higher, especially IL-6 but were not significant, compared with the control group. Cord blood levels of IL-6 in the cerebral palsy without asphyxia, sepsis, PROM, RDS and pneumonia were significantly different, compared with the control group.

Conclusion : Cord blood levels of IL-1¥â, IL-6 and TNF-¥á in the cerebral palsy group were increased, but concentrations of IL-6 increased significantly. Cord blood levels of IL-6 may be a useful value to predict the development of cerebral palsy, because they are related to IL-6 rather than to TNF-¥á & IL-1¥â in this study.
