Abstract
목 적 : 경우내경정맥 간생검의 유용성과 합병증에 대하여 알아보고자 하였다.
대상 및 방법 : 3년간 21명의 환자를 대상으로 하였다. 우측 내경정맥을 통하여 우간정맥까
지 9F Sheath를 넣고 Mansfield forcep을 이용하여 생검을 실시하였다.
결 과 : 경정맥생검을 실시한 이유는 과량의 복수 6명, 혈액응고기전의 이상 9명, 혈소판
감소 5명 및 기타 1명이다. 20명에서 조직을 얻었으며 이중 17명에서 병리적 진단이 가능하
였다. 3명에서는 얻은 조직이 작아 병리적진단에 적합치 않았다. Budd-Chiari 환자 1명에서
는 도관으로 간정맥을 선택하지 못하여 조직을 얻지 못했다.
합병증으로는 1명에서 천자부위에 혈종이 생겼고 4명에서 24시간 이후까지 천자부위에 동
통을 호소하였다.
결 론 : 경우내경정맥 간생검은 경피적간생검의 금기환자에서 비교적 안전하고 유용한 방
법으로 사료된다.

Purpose : To evaluate the usefulness of transjugular liver biopsy(TJLB), and possible
complications.
Materials and Methods : During a three-year period, TJLB was performed in 21
patients. Though the right internal jugular vein, a 9F sheath was introduced into the
right hepatic vein, and using Mansfield forceps, biopsy was then performed.
Results : Reasons for TJLB included massive ascites in six patients, coagulopathy in
nine, thrombocytopenia in five, and other causes in one. In 20 cases, we obtained liver
tissue, and pathologic diagnosis was possible in 17 ; in three cases, the amount of
biopsy tissue obtained was insufficient for pathologic diagnosis. In one patient with
Budd-Chiari syndrome, we were unable to catheterize the right hepatic vein and failed
to obtain liver tissue. Complications occurred in feve patients, as follows : hematoma
formation at the puncture site in one patient, and pain at the puncture site, lasting 24
hours, in four.
Conclusion : TJLB may be a safe and useful method in patients in whom percutaneous
liver biopsy is contraindicated.
