Abstract
목 적 : Paraquat 중독환자에서 환자의 예후와 고해상 단층촬영의 폐침범 정도 및 다른 임
상인자들과의 상관관계를 알아보고자 하였다.
대상 및 방법 : Paraquat 음독으로 고해상 CT를 시행한 총 41명의 환자 중 고해상 단층촬
영에서 이상소견을 보였던 31명을 대상으로 하였다. 고해상 단층촬영은 음독후 평균 7.8일
(1-21일)에 시행되었고 이 기간동안 본원의 기본 치료 방침에 따라 치료가 시행되었다. 음
독량, 연령, 그리고 혈중 백혈구수의 임상인자들과 단층촬영에서 폐침범 정도에 대하여 사망
자군과 생존자군간의 차이를 통계적으로 분석하였다.
결 과 : 고해상 단층촬영에서 이상소견을 보였던 총 31명의 환자중 11명이 사망하였고 20
명은 생존하였다. 사망자군의 폐침범 정도는 72.3±16.3%이었고, 생존자군의 폐침범 정도인
14.8±4.8%과 비교하여 통계학적으로 유의한 차이를 보였다(P<0.05). 사망자군의 연령은 25
±8.9세(16-41세)로 생존자군의 연령인 37.5±13.5세(11-67세)와 비교하여 젊었으며 유의한
차이를 보였다(P<0.05). 혈중 백혈구수와 음독량은 두 군간에 유의한 차이가 없었다
(P>0.05).
결 론 : Paraquat중독환자에서 HRCT의 폐침범 정도는 중독 정도 및 예후를 예측하는데유용한 방법으로 생각된다.

Purpose : To evaluate the prognosis of patients with paraquat poisoning by measuring
the extent of lung involvement, as seen on HRCT.
Materials and Methods : Forty-one patients with paraquat poisoning were treated
according to our hospital's routine protocol and HRCT scanning 1-21(mean 7-8) days
later. In 31, the results were abnormal, and these were retrospectively analysed.
Differences in the extent of lung involvement, patient age, ingested amount of paraquat,
and blood WBC count were compared between the group of survivors and those who
died.
Results : Among the 31 patients with abnormal HRCT findings, 11 died and 20 patients
survived. The extent of lung involvement among the group of survivors was 14.8±
14.8% ; among the deceased group, it was 72.3±16.3%. The age of the survivors was
37.5±13.5%(11-67) years, while that of the deceased was 25±8.9(16-41) years. Those
who died showed a significantly higher extent of lung involvement than the survivors,
and were younger(P<0.05). There was, however, no significant difference in blood WBC
count and ingested amount of paraquat between the two groups(P>0.05).
Conclusion : In paraquat poisoning, the extent of lung involvement on HRCT, is useful
for prediction of the prognosis and severity of poisoning.
