Abstract
목 적 : 속립성 폐결핵환자에서 단순흉부촬영상 속립성 결절의 연속 변화를 알아보고자 하
였다.
대상 및 방법 : 항결핵제로 치료후 완치된 13명의 속립성 폐결핵 환자에서 단순흉부촬영상
속립성 결절의 연속 변화를 후향적으로 분석하였다. 소아가 두 명이었고 성인은 11명이었으
며 연령분포는 2개월에서 73세(평균 38세)까지였다. 속립성 폐결핵으로 진단받은 후 5개월에
서 15개월(평균 10개월)까지 단순흉부촬영으로 추적하였으며 3명의 환자에서는 단순흉부촬
영상 결절의 완전소실후 고해상 CT를 시행하였다.
결 과 : 1주후 추적 촬영한 9명중 8명에서(89%) 그리고 한 달 내에는 모든 환자에서 속립
성 결절의 수와 크기가 감소하였다. 속립성 결절의 소실 기간은 평균 6.3개월이었으며 이 중
소아는 3.5개월(2-5개월)이었고 성인은 6.8개월(3-10개월)이었다. 고해상 CT를 시행한 3명
모두에서 속립성 결절은 완전히 소실되어 보이지 않았다.
결 론 : 속립성 폐결핵환자에서 단순흉부촬영상 속립성결절의 크기와 수는 적절한 치료 후
한달 내에 감소하기 시작하였으며 완전히 소실되는데 걸리는 기간은 평균 6.3개월이었다.
Purpose : To evaluate sequential changes in miliary nodules, as seen on chest
radiographs in patients with miliary tuberculosis.
Materials and Methods : We retrospectively analyzed sequential changes in miliary
nodules, as seen on the chest radiographs of 13 patients with miliary tuberculosis who
recovered completely ofter antituberculous medication. Two were children and 11 were
adults, and their ages ranged from 2 months to 73 years(mean, 38 years). In cases in
which miliary tuberculosis had been diagnosed from initial chest radiographs, follow-up
chest radiographs were obtained 5 to 15(mean, 10) months later. After complete
resolution of miliary nodules, as seen on chest radiographs, high-resolution CT scanning
was performed in three patients.
Results : As seen on follow-up chest radiographs obtained at one week, the number
and size of miliary nodules had decreased in eight of nine patients (89%), and on those
obtained at one month, these decreases were seen in all 13 patients. The mean duration
of complete resolution of miliary nodules was 6.3 months ; in childrens, this was 3.5
(range, 2-5) months, and in adults, 6.8 (range, 3-10) months, In all three patients
involved, high-resolution CT scans obtained after complete radiographic resolution of
miliary nodules showed no recurrence.
Conclusion : In patients with miliary tuberculosis, the size and number of nodules had
decreased within one month of adequate chemotherapy and on chest radiographs,
complete resolution was seen at 6.3 months, on average.
