소아에서 발생한 사골동 골종 1예

Abstract
골종은 부비동에서 발생하는 양성종양 중에서 가장 높은 빈도를 차지하나, 주위조직을 압
박하거나, 부비동 자연공을 폐쇄하여 부비동염 등의 합병증을 일으키기까지는 대부분이 무
증상이며 방사선 검사 상 우연히 발견된다. CT를 포함한 부비동방사선 촬영 상 0.25%에서
발견되며 대부분이 전두동에 호발하고 전두동 이외의 부비동에서는 드물게 발생한다. 비교
적 젊은 연령층에서 발생하나, 소아에서 발생한 경우는 드문 것으로 알려져 있다.
최근 저자들은 현재까지 대한 이비인후 과학회지에 보고된 예와 비교하여 가장 낮은 연령
인 12세 남아의 우측 사골동에서 발생한 거대한 골종 1예를 측비절개술 및 비외사골동 절제
술로 제거하여 좋은 결과를 얻었기에 문헌적 고찰과 함께 보고하는 바이다.

Osteoma is the most common benign tumor in the paranasal sinuses and the frontal
sinus is the most common site, whereas a little in ethmoid and maxillary sinus. Excision
of tumor is indicated when it is large or producing symptoms such as tearing and
exophthalmos.
Our case was managed via external ethmoidectomy approach, and the patient has been
doing well postoperatively without any complication. The external ethmoidectomy
approach is considered to be the route of choice for removal of large osteoma of the
ethmoid extending into the orbit.
