Abstract
암 위험군층을 대상으로 노년층의 건강진단과 동시에 계속적인 검사, 추후 관찰을 위한 기본 자료 조사를 통하여 종양표지자의 유용성을 규명하기 위하여 종합검진을 위해 내원한 환자 및 노인검진 환자를 대상으로 7개의 독립적인 종양표지자를 측정하고 종양표지자의 종합검진에서의 효용성을 검토하고 다음과 같이 요약한다. 1. 종합검진에서 AFP, CEA, CA125, PSA의 screening 유용성은 없었다. 2. 종합검진에서 일차적으로 광범위 종양표지자로 스크리닝을 하고 양성인 경우만 이차적으로 장기특이성 종양표지자의 조립분석이 유용하다. 3. 일차적 screening의 수단으로 Giftec, polyamine, LDH 등의 광범위 종양표지자의 조립분석이 유용하다. 이상의 성적을 종합하면 조기암 검진을 목적으로 하는 별도의 프로그램을 운영하여 위험군의 검출이 가능한 각 인자의 조립분석을 전향적으로 실시하거나, 종합검진의 경우에는 일차적으로 광범위 종양표지자 및 관련인자의 결과를 토대로 위험군에 대해 암특이성 종양표지자의 이차적인 시행을 실시함이 인적, 경제적 부가가치가 높을 것으로 사료된다.
To evaluate the effectiveness of tumor markers by 7 independent tumor markers for early detection of risk group, we surveyed the data for the geriatric health screener and the continuous follow-up test in the patients who visited the health promotion center of Pusan national university hospital and summarized as follows. 1. In the health screening, there is no evidence of effectiveness of AFP, CEA, CA125, and PSA for the screening. 2. It is useful to screen primarily with broad-spectrum tumor markers and then, in case of positive results, to perform the profile of the organ-specific tumor markers.
3. As a tool of primary screening, combination assay of the broad-spectrum tumor markers like Giftec, polyamine, and LDH is more effective. To summarize the results above and literature reviewed, it is suggested that combination assay of broad-spectrum tumor marker is more suitable for the early detection of risk group and carry out the organ specific tumor markers in the risk group selected based on the results of the primary broad-spectrum tumor markers and associated factors by consequent order. It will be benefit for financial and personal aspect.
