중합효소 연쇄반응 검사를 이용한 헬리코박터 파이로리의 검출-기존 진단법과의 비교-

Abstract
요약
목적 : 헬리코박터 파이로리는 위염과 소화성 궤양 발생의 중요한 원인균으로 알려져 있
고 위암과의 연관성도 인정되고 있다. 최근 다양한 항생제 병합요법으로 우수한 치료 성적
을 보이게 됨에 따라 정확한 진단과 적절한 치료가 중요시됨은 물론이고 치료 효과의 판정
을 위해서도 정확도가 높은 진단 방법의 개발이 절실하게 되었다. 중합효소 연쇄반응 검사
(PCR assay)는 매우 높은 민감도와 특이도로 여러 임상진단 영역에서 사용되고 있는 방법
으로 헬리코박터 파이로리에 대해서는 이미 국내에서도 검사가 진행 중이다. 저자들은 중합
효소 연쇄반응 검사와 기존의 검사 방법(rapid urease test, 배양검사, 조직학적 검사)의 검
사 결과를 비교하여 헬리코박터 파이로리의 진단 방법으로서의 유용성을 확인하였다.
대상 및 방법 : 96년 4월에서 97년 2월 사이에 위장관 증상으로 위장 내시경을 시행한 환
자 중 항 헬리코박터 파이로리 치료를 받은 과거력이 없는 193예에서 위 전정부와 체부에서
각각 겸자 생검을 시행하여 rapid urease test (CLO test)와 중합효소 연쇄반응 검사, 배양
검사, 조직학적 검사를 시행하였다.
결과 : 대상이 된 환자는 남자 94예, 여자 99예로 평균 연령은 51.5(12.9세였다. 이들 환자
중 111예에서 감염이 확인되었다. 각 검사 방법의 결과에 따라 얻어진 민감도와 특이도, 양
성 예측도, 음성 예측도는 다음과 같다. 민감도는 조직학적 검사, rapid urease test, 배양검
사, 중합효소 연쇄반응 검사에서 각각 86.5%, 95.5%, 73.9%, 98.2%로 중합효소 연쇄반응 검
사에서 가장 높았으며, 특이도는 각각 98.8%, 74.4%, 100%, 95.1%로 배양검사에서 가장 높
았다. 또한, 양성 예측도는 조직학적 검사, rapid crease test, 배양검사, 중합효소 연쇄반응
검사에서 각각 99%, 83.5%, 100%, 96.5%로 배양검사에서 가장 높았고, 음성 예측도는 각각
84.4%, 92.4%, 73.9%, 97.5%로 중합효소 연쇄반응 검사에서 가장 높았다. 각 검사 중2가지
이상 양성시 헬리코박터 파이로리 감염이라고 하였을 때 각 검사와 일치율은 조직학적 검
사, rapid urease test, 배양검사, 중합효소 연쇄반응 검사에서 각각 0.83, 0.72, 0.71, 0.94였으
며, 3검사 이상에서 양성시 감염이라고 하였을 경우에는 각각 0.86, 0.68, 0.77, 0.90으로 중합
효소 연쇄반응 검사에서 가장 감염 판정에 일치하였다(Kappa, chisquare test, SPSS ver.
7.0, USA).
결론 : 중합효소 연쇄반응 검사는 기존의 헬리코박터 파이로리 검사법에 비해 높은 민감
도와 유용하게 사용될 수 있는 특이도를 나타내고 있어 기술적인 뒷받침과 시설 등의 조건
이 구비된다면 헬리코박터 파이로리의 일차 진단과 치료 판정에 유용한 검사방법으로 사료
된다.

Background/Aims : The detection of Helicobacter pylori(H. pylori) both before and
after treatment is very important, but conventional methods to detect H. pylori are less
specific. The aim of this study was to evaluate the efficiency of polymerase chain
reaction (PCR) assay as a diagnostic method of H. pylori infection.
Methods : One hundred ninety three patients were included in the study, Their biopsy
specimens obtained from gastric antrum and body were submitted to PCR assay, rapid
urease test (CLO test), culture, and histological examination.
Results : Fifty eight percent (111 out of 193 patients) of the patients had H. pylori
infection. The diagnostic sensitivity/specificity of PCR assay, rapid urease test,
histological examination, and culture were 98.2%/95.1%, 95.5%/74.4%, 86.5%/98.8%, and
73.9%/l00%, respectively. The positive predictive value/negative predictive value of PCR
assay, rapid urease test, histological examination, and culture were 96.5%/97.5%,
83.5%/92.4%, 99%/84.4%, and 100%/73.9%, respectively The highest sensitivity was
achieved in the PCR assay, while the highest specificity was obtained in culture.
Conclusions : PCR assay was proved to be the most sensitive test with high
specificity for the detection of H. plyori.
